Objective: To explore the etiology and microsurgery treatment efficacy of primary trigeminal neuralgia. Methods: Use suboccipital sigmoid sinus approach, according to intraoperative detection whether the trigeminal nerve compress by vascular, then taken microvascular decompression. Result: Treated 36 cases, Cured or improved efficiency rate was 94.4% (34 cases), Postoperative pain did not ease the rate was 5.6% (2 cases) , Recurrence rate was 2.8% (1cases) , Postoperative severe complications rate was 2.8% (1cases), conclusion: Prim for trigeminal neuralgia is vascular compression, micro-vascular decompression surgery is a minimally invasive, fewer complications, effective treatment method. To remove pathogenic factors near the trigeminal nerve root is essential for improving the curative effect of the surgical treatment.
