BackgroundModern lifestyle with abundant nutrient supply and reduced physical activity, hasresulted in dramatic increases in the rates of type 2 diabetes mellitus(T2DM).It wasestimated that there were approximately 150 million patients of T2DM in the worldby the year of 2000, the prediction showed that the number of T2DM could bedoubled to 300 million by the year of 2025. T2DM is a lifetime disease and influencepatients' quality of life gravely, so T2DM has been one of the most hot study fields allover the world.T2DM is a metabolic disturbance syndrome, hypergly-caemia and insulinresistance(m) is it's key pathological fetures, it belong to the category of XIAOKEin traditional Chinese Medicine.most patients have polyphagia,thirsty,polydipsia,hyperdiuresis and Decreased body weight.Chinese medicine has longly history andaffluent experience on T2DM, for the past few years, the studies of Chinese medicinetherapy of T2DM have gained a lot of achievements and show us the feasibility andsuperionity of Chinese medicine on T2DM thoroughly.PurposeTangyimin is a natural Chinese medicine and bas been confirmed effective onT2DM in clinical application, we designed this esperiment to explore the effect and themechanism of tangyimin on type 2 diabetes mellitus in vivo and ex vivo.MethodThe type 2 diabetes mellitus rat models were established by high fatty and highsugar diet with low dose STZ injected intraperitoneally.After treated with Tangyiminfor 4 weeks,the fasting blood glucose level (FBG),blood fat and serum insulin level(FINS)were detected, calculate insulin sensitivity index, morphologic changes ofpancreatic islet were detected immunohistochemistry.At the same time,Isolating andculturing the rat pancreatic islets in vitro. Isolated islets were divided into 5 groups:(1)Control group;(2) FFA group;(3)FFA+Tangyimin group(including three different dose groups).After being cultured for 48 hours, then made a glucose-stimulated insulinsecretion test respectively, examined insulin level in every group by radioimmunoassayafter stimulated by low glucose and high glucose and compare them.Apoptotic cellswere identified by TUNEL method.Resultcompare with model group,the blood glucose, blood grease of the type 2 diabetesmellitus rats were prominently decreased and insulin sensitiveity index increasedsignificantly after Tangyimin; There were more islet cells and more immunohistochem-ieally-stained insulin areas per islet in the Tangyimin group in comparison with thosein mode.else compare with control group,the insulin level of the other groups afterstimulated by low glucose and high glucose was reduced significantly(P<0.001)andthe proportion of the apoptotic cell of rat islet cell was increased ignificantly(P<0.001).compare with FFA group,the insulin level of the low and the middle dose Tangyimingroups after stimulated by low glucose and high glucose wasn't significant difference(P>0.05),but the insulin level of the high dose Tangyimin group was increasedsignificantly(P<0.05) the high and the middle dose Tangyimin decreased theproportion of the apoptotic cell of rat islet cell as compared with FFA group but thelow dose Tangyimin hasn't effect.ConclusionTangyimin could abase the blood glucose and blood-fat,decrease IR andincrease islet cell and insulin of pancreatic, in addition, Tangyimin could also improveinsulin secretion of islet cell which was damaged by free fatty acids. we presume thatthe dffects were related to promoting islet cell regeneration, cutting down lipotoxicityand protecting the islet cell.
