Study On The Relationship Between ACE, CYP11B2 Genes Polymorphism And Essential Hypertension Complicated By Ischemic Cerebrovascular Disease

Objective: To investigate the relationship of gene polymorphism of angiotension converting enzyme(ACE), aldosterone synthase(CYP11B2) with essential hypertension(EH) and ischemic cerebrovascular disease(ICVD) in hypertensive patients. To filtrate out the sensitive genes and genotypes that associated with EH and ICVD.Methods: 297 ICVD patients,324 simplex essential hypertensives(SEH) and 305 controls were collected, ACE I/D polymorphisms and CYP11B2 -344T/C polymorphisms were detected with Polymerase Chain Reaction(PCR) and Restriction Fragment Length Polymorphism(RFLP), clinic datas were analyzed by Analysis Of Variance, genotype frequency were analyzed by Chi-square Test, Logistic Regression were used to filtrate risk factors of ICVD, and estimate odd ratio of every combined genotype.Result: 1.Age, Systolic Blood Pressure(SBP), (Diastolic Blood Pressure)DBP, Body Mass Index(BMI), Cholesterol(CHOL),and Glucose(GLU) of ICVD group were significantly higher than that of SEH group and controls, High Density Lipoprotein (HDL) of ICVD group was significantly lower than that of SEH group and controls. SBP, DBP, BMI of SEH group were significantly higher than controls. Age, CHOL, GLU and HDL in SEH group of and controls showed no significant differerce. Triglyceride(TG) of ICVD group and SEH group were significant higher than that of controls. TG in ICVD group and SEH group showed no significant difference. Sex distribution and Low-Density Lipoprotein(LDL) in the three group show no significant difference. 2.ACE gene showed a significant difference in DD genotype and D allele frequencies distribution in the three groups, DD genotype and D allele frequencies in ICVD group were significantly higher than those in SEH group and controls, but between SEH group and controls DD genotype and D allele frequencies distribution showed no significantly difference.3.CYP11B2 gene showed no significant difference in genotype frequencies distribution in the three groups.4.The two gene show a significant difference in combined genotype frequencies distribution in the three groups, DD-TC/CC combined genotype frequencies in ICVD group were significantly higher than those in SEH group and controls, no significant difference in combined genotype frequencies was showed between SEH group and controls.5.Logistic regression showed that after adjusting other confounding factors, Age, BMI, CHOL, GLU, and ACE gene DD+CYP11B2 gene TC/CC combined genotype were independent risk factors associated with ICVD in EH patients after Logistic regression analysis. In all the combined genotypes, DD-TC/CC has the highest risk to suffer ICVD.Conclusion: 1.The I/D polymorphism in ACE gene was significantly associated with ICVD, but DD genotype was not independent risk factors associated with ICVD in EH patients after Logistic analysis.2.There was not significant association between the -344T/C polymorphism in CYP11B2 gene and EH, ICVD.3.Logistic regression showed that Age, BMI, CHOL, GLU, and ACE gene DD+CYP11B2 gene TC/CC combined genotype were independent risk factors associated with ICVD; To prevent beforehand, the EH patients, especially the elder one with ACE gene DD+CYP11B2 gene TC/CC combined genotype, should be treated actively for their obesity, hyperlipemia, and diabetes.4.there may exist co-effective action between ACE gene and CYP11B2 gene, compared with other combined genotype, ACE gene DD+CYP11B2 gene TC/CC combined genotype significantly increased risk of ICVD in EH patients, odds ratio was 2.486(1.405,4.398).
