| Analysis of the curative effect and prognostic factors forpatients receiving radical resection with alpha-fetoprotein-negative hepatocellular carcinoma.Objective: To evaluate the curative effect and prognosticfactors for patients receiving radical resection with(AFP)alpha-fetoprotein-negative hepatocellular carcinoma(HCC).Methods: To compare 90 patients with AFP-negative HCC(groupA) to 80 patients with AFP-positive HCC (group B) treated inour hospital from December 1996 to March 2005. The radicalresection was performed in all the 170 patients.Result: Median follow-up time was 42.6 months; follow-up ratewas 94.7ï¼…. In group A, The cumulative survival rate and thedisease-free survival rate of 1,3,5-year were 90.5ï¼…,75.1ï¼…,52.6ï¼…and 78.1ï¼…,41.1ï¼…,24.7ï¼…respectively: Intrahepaticrecurrence in 51. among them, 23 patients recurring within 2years; 6 patients with postoperative metastasis (7.1ï¼…). Ingroup B, The cumulative survival rate and the disease-free survival rate of 1,3,5-year were 93.4ï¼…,66.9ï¼…,43.7ï¼…and66.7ï¼…,41.9ï¼…,30.3ï¼…respectively; 49 with intrahepatic recurrence. 32 recurring within 2 years; 11 had postoperativemetastasis (7.2ï¼…). Overall survival, tumor-free survival, re-currence and metastases rate without significant between thetwo group. 2-years recurrence rate in group A was lower thangroup B. By univariate analysis, Five variables includinghepatic cirrhosis, tumor grade, tumor-embolus, TNM classi-fication, metastases were related to overall survival rate. Coxproportional hazards model study showed that hepatic cirrhosis,tumor grade were significant independent prognostic factors.univariate analysis indicated tumor size, number of tumor,tumor-embolus, tumor grade, capsule state, postoperative AFP,TNM stage were associated with tumor-free survival.Multivariate Cox Regression tests indicated that TNM stage,tumor-embolus, postoperative AFP were the factors affectingdisease-free Survival.Conclusions: Up to the time of following up, the cumulativesurvival rate, the disease-free survival rate were nosignificant between group A and group B. Early recurrence ratewas lower in group A. Hepatic cirrhosis, tumor grade weresignificant independent prognostic factors. TNM stage,tumor-embolus, postoperative AFP were important prognosticfactors for tumor-free survival. |
