BackgroundDespite decades of research and a vast literature devoted to the topic of woundhealing, the pathophysiology of keloids remains incompletely understood. Sometimeseven a slight injuring such as injecting can cause the development of keloids. Thepatients of keloids often have a long and instability course of disease and thisabnormal scar likes to infringe in the special position of body and its multiplicationalways keep in high rate. The clinic workers try a lot of therapies to cure the specialdisease, however, there is no excellent therapy to relieve the pain of patients nomattersurgical excision or adjunctive therapies by now. Keloid is not a kind of cancer, butthe biological characteristics of keloid take a large amount of trouble to the patients.As a result, the efficiency prevention of keloids is significantly important. Althoughthe pathophysiology and clinic therapy of keloids have a fast development in theresent years, there are no ways to inspect the susceptibility to keloid and risk ofdisease. If there is an easy way to pick up the keloid predisposition and give anefficient therapy to them, the pain will be lessen. That needs further studies of course.Our study group has proved that the genotype polymorphism could be found incodon 72 exon 4 of p53 gene which was picked up from the lymphocyte in peripheralblood. And the important one is the genotype polymorphism has the differentiabetween the normal human and keloid patients. This differentia was so important that it can afford the theoretic and experimental evidence for keloid predispositiondetection in clinic. Also, the keloid detected reagent kit was manufactured on the baseof the differentia for clinical application.Objective and Clinic SignificanceThe aim of our study is applying the kit to examine the large-scale clinic sampleto validate the sensitivity and specificity on the base of the past studies, as well as, tovalidate if the kit can be used in large scale detection in clinic. The method ofdetecting the keloid susceptibility must be established to prevent the unnecessarysurgical operation and the assistant therapy after the surgical treatment.The application of the reagent kit made the keloid predisposition preventionpossibly. The keloid predisposition can be advised to avoid the dangerous action andsensitive things to reduce the risk of keloid if he has a positive result. Also, the clinicworkers are easy to find out the keloid predisposition and give them the integranttherapy before the surgical operation.Samples and MethodsBlood samples of keloid were taken from 61 patients which were approvedgetting keloid by at least two clinic workers. Because the test is an random-controlexperiment, so we made up an normal group compared with the keloid group. Age,raceand gender in two groups were similar and the normal one has none abnormal scarhistory. At the same time, two groups have no kin and tumor history. Course: we getthe human genome DNA from the blood sample and desire the primer and probe ofp53 gene. The codon-72 polymorphism of the tumor suppressor gene p53 weredetected by using polymerase chain reaction and reverse dot blot and the result can beshowed through three different coloured firm. ResultsThe genotype of the 122 sample just like this: Proline/ Proline genotype inkeloid group has 38 cases(62.3% in total); Proline/Arginine genotype in keloidgroup has 12 cases(19.67% in total); Arginine/Arginine genotype in keloid grouphas 11 cases(18.03% in total). Proline/Proline genotype in control group has 21cases(34.43 % in total); Proline/Arginine genotype in control group has 25cases(40.98 % in total); Arginine/Arginine genotype in control group has 15cases(24.59% in total).The result of statistics analyse:There are 59 Proline/Prolinegenotype cases and 63 not Proline/Proline genotype cases in 122 samples. Comparedwith the clinic diagnosis, the sensitive of the kit is about 62.3%(38/61) and thespecificity of the kit is about 65.6%(40/61). The positive predictive value of kit is4.15/1000 and the negative predictive value of kit is 998/1000. It is reported that theprevalence of xanthous in Asia is about 2.3/1000. So, we can tell a truth that thehuman who has a Proline/Proline genotype in codon-72 of the tumor suppressor genep53 is 2-3 times more danger than the others. The OR value is 3.429.The codon-72 polymorphism of the tumor suppressor gene p53, theProline-encoding allele and Proline/Proline genotype are the genetic predisposition tokeloid of the Han race in Guang Dong, China.The keloid reagent kit can pick up thegenetic predisposition to keloid among the human beings and will be a assistant wayto forecast the disease.ConelutionThe keloid-testing reagent kit is long with fast checking, convenient handlingand less cost. Only 1ml blood and little time can get the answer of the test. The kitcan help the clinic workers and keloid predisposition human beings fight back theattacking of the keloid. The sensitive of the kit is about 62.3% and the negative predictive value of kit is 998/1000 can make the reagent kit be a available test forpredisposition forecasting. In a word, it is a good kit which can be use efficiently inthe clinic condition.
