Investigation On Levels Of Plasma Adiponectin In Polycystic Ovarian Syndrome And Their Relationships With Insulin Resistance

Objectives: Observing the levels of glucose, insulin in oral glucose tolerance (OGTT) ,fasting total adiponectin(APN) and fasting total adiponectin/high molecular weight adiponectin(HMWR) in subjects with Polycystic ovarian syndrome(PCOS).To investigate the relationship between APN and insulin resistance(IR) in PCOS.Methods: Case-control study. Eighty-five PCOS and twenty-nine control subjects were included in the study. Women with PCOS were classified as hyperinsulinism (HI) (n=29) or non-HI (n=85) on the basis of fasting insulin levels. (1) Plasma levels of gonadotropins and ovarian steroids were deternined. Oral glucose tolerance test (OGTT) and insulin releasing test(IRT) were performed and homeostasis model assessment insulin resistance index (HOMA-IR) was calculated.(2) Blood sample were obtained at fasting and lasma were isolated. APN were analysed by enzymatic immunoassay and high weight molecular adiponectin/total adiponectin were analysed by immunoblotting.Results: (1) There was the inversely correlationship between APN and WHR, lnHOMA-IR(r=-0. 39,-0.52)(P<0. 05)in subjects with PCOS,and the same correlationship we could obtain in total subjects (r=-0. 36,-0. 45) (P<0. 05).(2) APN was not an independent protective factor of PCOS,but LH/FSH and lnHOMA-IR were independent risk factors.The OR were 1.51 (95%CI: 1.16-1.96) and 1.26 (95%CI: 1.10-1.44) (P<0. 05).(3) APN was an independent protective factor of PCOS with HI,the OR was 0.81 (95%CI: 0.67-0.97) (P<0. 05). WHR was an independent risk factor,the OR was 8.57 (95%CI: 2.14-34.30) (P<0. 05).(4) APN was an independent factor of FINS and lnHOMA-IR (β=-0.22,-0.02) (P<0. 05).(5) We could not obtain the strips of low molecular weight adiponectin,middle molecular weight adiponectin and high molecular weight adiponectin with immunoblotting . Using Immunoprecipitation before immunoblotting may be helpful in the reseach latterly.Conclusion: APN may be the factor involveing in the pathogenesis of insulin resistance(IR) in PCOS subjects.
