Study DesignTo collect the history of the patients who were diagnosed as burst fracture. During the course of operation we collect the superior and inferior disc end-plates cartilage ,using histological method to observe the changes of important inflammatory mediator NO, iNOS and NF-KB which was express in post-trauma end-plate cartilage.ObjectiveTo compare the injury extents of superior and inferior cartilage end-plates of burst fracture and discuss the possible pathologic mechanism of the post-traumatic disc changes.BackgroundA new messenger numerator in the cell ,NO has simpleness configuration, strongly reactivity and instability.NO indwells tissue within organism and concerned with process of physiology and pathology. Lately, reports was described about NO concerned with disc degeneration mechanism.But,how is it in Post-traumatic end-plates cartilage? Whether have difference between post-traumatic superior disk and post-traumatic inferior disk?MethodsTo collect 49 patients' history information. In the operation we collect the superior and inferior end-plates cartilage of burst fracture respectively.And to collect 18 normal end-plates cartilage from 3 fresh bodys. Mensurate the content of NO in the end-plates cartilage.(1) To analyse the difference between post-traumatic superior disk end-plate cartilage and normal end-plates cartilage.(2) To analyse the difference between post-traumatic inferior disk end-plate cartilage and normal end-plates cartilage.(3)To analyse the difference between post -traumatic superior disk inferior end-plate cartilage and inferior disk superior end-plate cartilage.By using immunohistochemistry, we observe the expression features of two important proteins : inflammatory mediator iNOS and NF-KB in the end-plate cartilage(1) To analyse the difference of positive rates between the post-traumatic superior and inferior disk end-plate cartilage. (2)To analyse the difference of positive rates in post-traumatic inferior disk end-plate cartilage and normal end-plates cartilage.Results(1) NF-κB was expressed in the nucleu of cartilage cells. expres are more apt to injury in burst fracture, the post-traumatic superior disk end-plate cartilage more than the inferior. There is no obvious expressed in the normal end-plate cartilage. (2) iNOS was expressed in the nucleu of cartilage cells. expres are more apt to injury in burst fracture, the post-traumatic superior disk end-plate cartilage more than the inferior. There is no obvious expressed in the normal end-plate cartilage. (3) the content of NO in the post-traumatic superior disk end-plate cartilage are more than normal .(4)there is no difference of the content between inferior disc end-plate cartilage and normal(5) the content of NO in the post-traumatic superior disk inferior end-plate cartilage are more than inferior disk superior end-plate cartilage .(6) the positive rates of NF-κB and iNOS in the post-traumatic superior disk end-plate cartilage are mora than the inferior. (7) the positive rates of NF-κB and iNOS in the post-traumatic inferior disk end-plate cartilage are mora than the normal.Conclusion(1)By molecular pathologic measurement the superior disk end-plate are more apt to injury in burst fracture.(2) because there are few difference between the post-traumatic inferior disk end-plate cartilage and the normal, there are few injury in the post-traumatic inferior disk end-plate cartilage. (3) As mensurate NO and the different expressions of the two proteins between the degenerative disk and injury disk were concerned, we suspect that there exist some different pathologic mechanism while the disk was injuried.
