ForewordTB epidemic in recent years show an upward trend, and the mortality rate is thehighest infection rate of infectious diseases, the rapid diagnosis of tuberculosis is amajor global TB control measures. Current TB diagnosis Based on bacteriology andtuberculin test, Mycobacterium tuberculosis (mycobacrterium tuberculosis, MTB), andtheir growth characteristics Bacillus Calmette-Guerin (BCG) vaccination in China'sextensive use of these methods to make the diagnosis of tuberculosis is extremelylimited. ESAT-6 (early secretory antigrntic target-6) is a specific secretion of lowmolecular weight proteins, can stimulate the body produce specific immune globulin, using its combination of features detected by ELISA in serum antigen and antibodycontent, we can distinguish between MTB infection or the BCG vaccination. CFP-10(culture filtrate protein-10) The 1998 identified the low molecular weight proteins canbe specific recognition and the release of IFN-γ. Therefore, CFP-10 can be diagnosedas T-cell antigen. CFP-10 and ESAT-6 Joint will enhance sensitivity 90%, specificitywas 97%. Based on serum IFN-γof indirect detection CFP-10 and ESAT-6two-specific antigen of MTB in patients diagnosed tuberculosis, pneumonia andexcluded patients with other infectious diseases in patients with a normal expression, and with tuberculosis in experiments in the crowd in the detection and the twoexperimental methods of the special sensitivity of contrast, research and evaluation ofthe test in clinical diagnosis of feasibility. To inwestigate the two specific antigensESAT-6 and CFP-10 of MTB through IFN-γ. we use the test to distinguish the expressof two antigens from the three data of the pulmonary tuberculosis, pneumonia andnormal groups.Materials and Methods1. Case options: Selected Shenyang City thoracic hospital inpatients 60 cases, ofwhich 20 cases were diagnosed with pneumonia, 40 confirmed cases of tuberculosiscases, 23 cases were male, 17 female, aged between 16 to 79 years old. (All patients met the following criteria in patients with a newly diagnosed with tuberculosis drugshas not yet begun; Two patients not previously treated with tuberculosis drugs for morethan one year) normal control group of 20 cases selected tuberculin (PPD) test negativepeople, mainly children under the age of 10.2. Sample preparationAcquisition targets all subjects (TB patients in the tuberculosis treatment) fasting blood2ml, to 3,000/min 3 minutes centrifugal separation serum, -20℃preservation.3. Experimental Methods(1) ELISA①the 100ul different concentrations of serum and the standard reaction to the holein one hour at 25℃.②After washing three times, by adding enzyme-labeled antibody, at 25℃for onehour.③Rinse three times, by adding color liquid, 20 minutes later, in all holes havejoined 100ul terminated liquid, blending after typing in 450nm with the measuredabsorbance value of the pore solution.(2) PPD experimentalExperimental Methods: TST choice of the left side of the forearm in the third song, 0.1ml (5IU) intradermal injection, on the 26th with a long single 10mm short slantneedles and syringes boundaries, injection should be able to produce uplift Pichugin, border clearly can be seen clearly above the small concave. 48-72 hour after theexperiment results.Experimental Results1. By ELISA method to detect the three groups were the result of tuberculosis andpneumonia group P<0.01, tuberculosis group and the control group results of theanalysis P<0.01, in the normal group and the control group pneumonia results P>0.05.One group tuberculosis and pneumonia compared the sensitivity and specificity were92.5% and 80%, tuberculosis group and the normal group, the sensitivity andspecificity were 92.5% and 90%. Tuberculosis and pneumonia patients with the groupand the normal group were statistically significant, and pneumonia group and thenormal group, no statistical significance.2. Tuberculin test in TST a group that the three test results. Tuberculosis group and pneumonia group P>0.05, sensitivity is 95% and specificity of only 5%; Tuberculosisgroup and the control group comparison of P<0.01, Sensitivity and specificity are 95%and 100%; Group pneumonia and tuberculosis control in the group P<0.01, statisticalsignifieancelts.ConclusionThis experimental method through two pairs of three experimental group had thegroup TB infection in the test and comparison, and the test results were analyzedstatistically, the feasibility of this experiment to give a clear statement of theexperimental results show that ELISA method in the experimental group in the groupcontrol sensitivity specificity were generally higher than the PPD experimentalmethods and tuberculosis and pneumonia in the differential diagnosis of statisticalsignificance, that the experiment in the clinical differential diagnosis of tuberculosis isvaluable.
