The Analysis Of 103 Patients With Renovascular Hypertension

Objective: To analysis the renovascular hypertension (RVH) from etiological factors, clinical manifestation,auxiliary examination and treatment shares. Methods: 103 patients finally diagnosed as RVH from January, 2000 to 2007 were included in this study and performed with retrospective study. Results: Takayasu arteritis(TA), Atherosclerotic renal artery stenos (ARAS) and fibromuscular dysplasia (FMD) account for 39.8%, 27.2% and 17.5% reasons in all 103 RVH patients, and another 16 patients were derived from congenetial renovascular deformation combined with renal dysplasia (3.9%, 4 cases), renalvascular being oppressed by peripheral occupying lesion (4.9%, 5 cases), renovascular thrombus from surgical trauma (0.97%, 1 case ), embolism (0.97%, 1 case) and unclassified resons (4.9%, 5 cases) respectively. The age of first attack in TA and FMD was obviously lower than in ARAS. Renal ateriography was the golden standard to make rapid diagnosis. Abdominal vascular murmur in TA was more frequent than in ARAS and FMD . Most ARAS lesions were located in the debouch or 1/3 proximal of renal ateries,and majority stenosis is excentric; Most TA was located in the debouch of renal ateries,normally with more than 2 branches involved and regionally lesioned; FMD was often located in the middle or distal end of renal ateries and with characteristic of typical string-beads. Patients which got interventional therapy showed better prognosis than those got drug-treatment. Conclusions: TA, ARAS, FMD and shared most parts in RVH etiological factors, while ARAS was of increasing portion and interventional therapy seemed to be the key treatment for patients with RVH.
