Objective :By investigating expression of CD46 in human breast cancer,normal breast tissue and benign breast diseases and analyzing correlation intensity of CD46 and clinicopathological parameters, we try to explore the mechanism of tumor escaping immune defence and the importance of CD46 in clinical research.Methods: Patient with breast cancer,normal breast tissue and benign breast diseases proved pathologically from 2004 to2005 and operated in the second people hospital of Guangxi were enrolled and divided into four groups. The breast cancer group include 53 cases of breast cancer tissue.The normal mammary gland group includes 15 cases of normal breast tissue. The hyperplasia of mammary gland group includes 17 cases of hyperplasia of mammary gland tissue. The fibroadenoma of breast group includes 17 cases of fibroadenoma of breast tissue. The immunohistochemical staining method (SP) was choosed to detect the expression and distribution of CD46 in all four groups. The correlated clinicopathological parameters of human breast cancer were collected. The data were analyzed by SPSS 13.0 for window.Result :1,The positive rate of CD46 was 96.2%,66.7%,70.6%,70.6% respectively in breast cancer tissue,normal breast tissue,hyperplasia of mammary glands tissue,fibroadenoma of breast tissue. By statistical analysis, the positive rate of CD46 in breast cancer tissue,normal breast tissue,hyperplasia of mammary glands tissue,fibroadenoma of breast tissue had obvious difference (P<0.05).The positive rate of CD46 in breast cancer was obviously higher than in normal breast tissue,hyperplasia of mammary glands tissue,fibroadenoma of breast tissue . The positive rate of CD46 in normal breast tissue,hyperplasia of mammary glands tissue,fibroadenoma of breast tissue hadn't obvious difference (P>0.05).2,The samples of breast cancer tissue were divided into several groups according to clinicopathological parameters and were compared to their intensity. The results showed: The positive intensity of CD46 in the second and the third pathological grade was obviously stronger than in the first pathological grade. The positive intensity of CD46 in positive expression of C-erbB-2 was obviously stronger than in negative expression. The positive intensity of CD46 in NPI higher than 3.4 was obviously stronger than in NPI lower than 3.4. There weren't obviously difference in several groups that divided into according to other clinicopathological parameters.3,The correlate analysis was done between the positive intensity of CD46 and clinicopathological parameters. The results showed: There were positive correlation between the histological grade,the positive C-erbB-2 expression,invasion of lympgatic vessel,NPI and the positive intensity of CD46(r value0.533, 0.403, 0.414, 0.311, P<0.05).Conclusions: 1,The positive rate of CD46 in breast cancer was obviously higher than in normal breast tissue,hyperplasia of mammary glands tissue,fibroadenoma of breast tissue .The result indicate this: The mechanisms of breast cancer escaping attack of complement and retaining immune tolerance are related with high positive rate of CD46. 2,The positive intensity of CD46 in the second and the third pathological grade was obviously stronger than in the first pathological grade. The positive intensity of CD46 in positive expression of C-erbB-2 was obviously stronger than in negative expression. The positive intensity of CD46 in NPI higher than 3.4 was obviously stronger than in NPI lower than 3.4. There were positive correlation between the histological grade,the positive C-erbB-2 expression,invasion of lympgatic vessel,NPI and the positive intensity of CD46.The result indicate the positive intensity of CD46 is related with prognosis of breast cancer. And what is more, the result likely demonstrate the positive expression of CD46 in breast cancer is closely related with genesis,and development,proliferation,differentiation of breast cancer. In summary, It is very important for estimating prognosis of patient with breast cancer by CD46 after realizing this in clinical work. Simultaneously, this research provides some important evidence for applying anti-CD46 or other related complement medicine as immunizing treatment for patient with breast cancer in clinic.
