Objective: To explore the correlation between serum IGF-1,MMP-9,TIMP-1 levels and ventricular remodeling and risks stratification in essential hypertensive patients.Methods:(1)Experimental group included 75 hypertensive patients hospitalized which were further subdevided into four groups :normal geometry (mean age59.57±14.43 years; 4 male, 3 female); concentric remodeling (mean age 70.13±10.23 years; 3 male,5 female); concentric hypertrophy with 30 patients (mean age 70.9±11.29 years; 17 male, 13 female); eccentric hypertrophy with 30 patients (mean age 72.23±12.23 years; 21 male,13 femal).8 patients without essential hypertension as control group (mean age 53.88±14.12 years; 5 male,3 femal).(2)ELISA measurement Plasma MMP-9, TIMP-1, IGF-1 levels were performed after at least 12 fasting hours,while at the same day echocardio graphy was performed to measure parameters of LVEF,LVIDd,LVIDs,LA,IVST,LVPWT, and LVMI,RWT were calculated according to these parameters. (3) According to the highest level of blood pressure in the prior history and risk factors, 75 hypertensive in patients again devided into three groups :Low,Moderate added risk (mean age59.57 years±14.43; 4 male, 3 female); high added risk (mean age 70.13±10.23 years; 3 male,5 female);very high added risk (mean age 70.9±11.29 years; 17 male, 13 female).Result:(1) Serum IGF-1,TIMP-1 levels in experimental group were significantly higher than those in control group (p<0.05), in contrast the MMP-9 in experimental group was significantly lower than that in control group (p<0.05).(2). Serum IGF-1,TIMP-1 levels in concentric hypertrophy were significantly higher than those in normal geometry in hypertensive patients(p<0.05—0.01), whereas the MMP-9 in concentric hypertrophy was significantly lower than that in concentric remodeling (p<0.05); Serum TIMP-1 level in eccentric hypertrophy was significantly lower than that in concentric remodeling and normal geometry.(3).The serum IGF-1,TIMP-1 levels in concentric hypertrophy were significantly higher than that in eccentric hypertrophy (p<0.05—0.01) ,whereas the MMP-9 in concentric hypertrophy was significantly lower than that in eccentric hypertrophy (p<0.05).(4). A significantly positive correlation was observed between IGF-1 and IVST,RWT (p<0.05); TIMP-1/MMP-9,TIMP-1showed the significantly positive correlation with IVST,LVPWT,RWT (p<0.05—0.01);MMP-9 showed the significantly negative correlation with IVST,LVPWT (p<0.05).(5). Very high added risk group and high added risk group showed a significant higher TIMP-1 level than that in Low,Moderate added risk group(p<0.05).(6). Very high added risk group showed a significant higher IGF-1 level than that in high added risk group and Low,Moderate added risk group(p<0.01),respectively.Conclusions: Present data demonstrate that:(1).There is a significantly correlation between serum IGF-1, MMP-9, TIMP-1 and different patterns of ventricular hypertrophy in essential hypertensive patients.(2). There is a closely correlation between serum IGF-1, MMP-9, TIMP-1 and cardiovascular risk stratification.
