Objective: That the more and more many body inner , extracorporeal experiment evidence indicate Epithelial to myofibroblast transition (EMT) is renal interstitial fibrosis(RIF)one of the main approach happening and leading to end last phase kidney function failure then of kidney tubule mesenchyme. Emerging evidence suggests that Integrin -linked kinase (ILK) is very crucial in transform growth factorβ(TGF-β)mediating EMT.To elucidate the association of the expression of ILK in human renal tubulointerstitial with renal interstitial pathological injury of patients with primary glomerulonephritis and to evaluate the role of Integrin-linked kinase in the genesis of chronic renal interstitial injury and fibrosis. Methods: eighty-eight specimens were obtained by percutaneous renal biopsy from patients with primary glomerulonephritis .According to tubulointerstitial injury under light microscopy, specimens were classcified into four groups: grade-1 :45 patients in minor injury group , grade-1:38 patients in middle injury group , grade-3: 5 patients in severe injury group . the cancer choosing 20 example kidneys cancer patient operation at the same time cutting off a specimen, 20examples the regular kidney organization mesenchyme nothing is damaged obviously, therefore thinking that the mesenchyme damages grade-0. All specimens were semiquantitatively examined expression of ILK and TGF-β1,α- smooth muscle actin(α- SMA) by immunohistochemisty EnVision . Statistical analysis software groups ILK and TGF-β1,α- SMA expression, and the relationship between the degree of renal interstitial injury and ILK,TGF-β1,α- SMA expression.Clinical indexes of these patients were also collected.Results: (1) ILK was rarely detected in control group, and the expression of ILK increased gradually in other groups with the degree of renal interstitial pathological injury (P<0.001). ILK was observed in tubulointerstitial space with severe lesion, with the most prominent in tubular epithelial cells, next tubulointerstitial spaces. A significant positive correlation was found between the expression of ILK in renal tubulointerstitial tissue and the degree of renal tubulointerstitial pathological injury (r=0.650, P<0.001). (2) TGF-β1 was rarely detected in control group, and the expression of TGF-β1 increased gradually in other groups with the degree of renal interstitial pathological injury (P<0.001). TGF-β1 was observed in tubular epithelial cells, Myofibroblast, next tubulointerstitial spaces. A significant positive correlation was found between the expression of TGF-β1 in renal tubulointerstitial tissue and the degree of renal tubulointerstitial pathological injury (r=0.855, P<0.001). (3) At normal control groupα- SMA Only expresses in the blood vessel smooth muscle , has very few expression the renal interrenal .Aggravates the expression enhancement along with the renal interrenal pathological change,In specific weight damage groupα- SMA widely distributes in the renal interrenal region,Some lumen destruction and the atrophy renal tubular epithelial cell cytoblastema also obviously shows, and decreased gradually with the degree of renal interstitial pathological injury (P<0.001),A significant positive correlation was found between the expression ofα- SMA in renal tubulointerstitial tissue and the degree of renal tubulointerstitial pathological injury (r=0.611,P<0.001). (4) ILK and TGF-β1α-SMA expression relations: Along with tubulus renalis between nature pathology extent of damage aggravation, ILK and TGF-β1α-SMA in the nature expression increases in the acorn tube between, the expression tendency is similar, The ILK expression and the TGF-β1 expression,theα-SMA expression in renal interstitial has the positive correlational(r=0.898,0.473 P<0.001 ),as well as the correlational between the TGF-β1 and theα-SMA expression(r=0.572,P<0.001). Conclusion: (1) ILK appear to be important in the genesis of renal interstitial fibrosis in patients with primary glomerulonephritis;(2) TGF-β1 appear to be important in the genesis of renal interstitial fibrosis A significant positive correlation was found between the expression of TGF-β1 andα- SMA.; TGF-β1 might induces renal interstitial fibrosis by EMT .(3) Prompts in the normal kidney organization not to have Myofibroblast nearly, but renal interrenal nature fibrosis and Myofibroblast quantity related .(4) In all degrees of renal interstitial pathological injury, A significant positive correlation was found between the expression of ILK and TGF-β1α- SMA. ILK may be participate in the formation of renal interstitial fibrosis, via the induction renal tubular epithelial to myofibroblast transformation (EMT).
