Objective: Subarachnoid hemorrhage is the main cause ofaneurysmai patients'death, it also results in high morbidity and mortality.Early accurate and definite diagnosis of such disease is important for thetherapy. We examined spontaneous SAH patients by using computertomography angiography (CTA) digital subtraction angiography (DSA),appreciate and discuss the practicability as well as reasonability ofintracranial aneurysm through the results analysis and compare. Thefeature of 3D-CTA and DSA imaging were re-analyzed, the practicalvalue of the two methods in diagnosis of aneurysm were discussed.Methods: 86 patients with SAH were exanimated by 3D-CTA andDSA with 3 days after the first attack, the imaging characteristic werestudied. The diagnoses accuracy and the depicture capability ofaneurysm's form, sizes and relation with ambient structures werecompared. The images of 3D-CTA,2D-DSA, and 3D-DSA were read byseparately by 3 groups of radiologists, each group had 2 doctors. Theaneurysm' s figure, the clearance of neck showing, relationship betweenaneurysm and parent artery, the aneurysm's relation with ambientstructures were compared.Results: 94 aneurysms were found in 74 patients. The advantage ofdepicturing aneurysm's relations with bony and ambient soft tissue is dramatic in 3D-CTA. DSA showed advantages in delineating irregularaneurysm and the aneurysm's relation with ambient arteries. CTAexamination had no complications, 2 patients had hematoma in puncturesite, but had no stroke or rupture of aneurysm. When compared with2D-DSA, 87 aneurysms of 94 showed the similar form in 3D-CTA, but 7aneurysms showed obvious differences; all images showed the similarresults in 3D-DSA. There was no difference in diagnosis accuracybetween 2D-DSA and 3D-DSA by statistic anylasis.Conclusion: Above all,the diagnosis capability of CTA is similar toDSA, meanwhile it can show the aneurysm's relation with ambientarteries, the luminal thrombosis and calcification, the feature of ambientbony structure, depicture the feature of aneurysm with accuracy. CTAcan be the premier choice in SAH patients. 3D-CTA may provideinformation for aneurysm clipping with lower expense and complications.DSA is replaceable in interventional treatment, excellent therapy mayachieve when the information of 3D-CTA, 2D-DSA, especially 3D-DSAanalyzed totally.
