Objective: To explore the diagnosis and microsurgical treatment of fourth ventricle tumors.Methods: Fourty-one patients sufering from fourth ventricular tumors were admitted to our hospital and underwent removal of tumor by midline suboccipital approach under microscope.The pathological characteristics,clinical manifestations,image changes,surgical operation, and prognosis were summarized and analyzed.Results: Among the 41 patients,the tumor was total removed in 35 cases(85.3%) ,subtotal in 6 cases(14.7%) and no patient was death in this group after the operation.The post -operative pathological examination showed medulloblastoma in 17 cases,ependy -moma in 13,choroids plexus papilloma in 4,glioblastoma in l,and meningioma in 1 case. The symptoms of intracranial hypertension and dystaxia were improved to some extent. During the period of hospitalization,2 cases suffered from pulmonary infection,2 cases stress ulcer, 1 cases intracranial infection, 1 cases tremor, 1 case respirary dysfunction, and 1 cases suffered from subcutaneously fluidify,These symptoms disappeared by antisympto matic therapy.Conclusions:CT and MRI are the main examinations for the diagnosis ofthe fourth ventricule.The Application of microsurgery,the protection ofthe brainstem function and the precaution of hydrencephalus caneffectively elevate operation quality for fourth ventricular tumorsexcision.
