Objective:To evaluate and compare the clinical results of putting pillow and pedicle screw fixation in the treatment of traumatic thoracolumbar vertebrae compression fracture. And to study the effective method.Methods: A total of 68 patients with thoracolumbar vertebrae compression fracture had putting pillow or pedicle screw fixation in department of orthopedics,zhangzhou Hospital of TCM,fujian province from July 2000-March 2006 were selected.Divided into two groups,one group was treatment by reduction by putting pillow.and the other was treatment by reduction of pedicle screw fixation.By reviewing the data of cases and the x-ray checking.the percentage of damaged vertebral height and change of Cobb angle were measured before and after treatment as well as follow-up after treatment.The data were analyzed by significant.Results: In putting pillow group,the, percentage of damaged vertebral height was 56.55±4.61 before treatment,and 77.23±4.76 after treatment, and 72.51±5.03 at final follow-up.There are very significant difference in them.The Cobb angle was 19.07±3.65 before treatment,and 11.01±1.96 after treatment, and 13.66±1.44 at final follow-up.There are very significant difference in them.In operated group, the. percentage of damaged vertebral height was 57.30±4.80 before treatment, and 92.42±2.97 after treatment, and 84.78±4.99 at final follow-up.There are very significant difference in them.The Cobb angle was 19.42±2.81 before treatment,and 5.98±1.57 after treatment, and 10.46±3.09 at final follow-up.There are very significant difference in them. the percentage of damaged vertebral height and Cobb angle in between groups was not significant difference before treatment, but There were very significant difference after treatment and final follow-up.Pain and work according to the method of Denis, There were very significant difference in between groups in final follow-up. Conclusion:(1)The putting pillow and pedicle screw fixation are effective and security for the treatment of traumatic thoracolumbar vertebrae compression fracture. (2)The pedicle screw fixation have an advantage in reposition and rectification fracture ,as well as improvement of pain and work. (3) The damaged vertebral height and Cobb angle manif lost when take out of the internal fixation.
