The Clinical Research Of Surgical Therapy And Therapeutic Effect Of Nasal Inverted Papilloma

ObjectiveTo discuss the therapeutic effect and indication of the three processes through the statistics of the recurrence rate of simple endoscope approach, endoscopic combined with Caldwell-luc and lateral rhinotomy approach. On the basis of diseased region and degree to recall the modus operandi of low recurrence rate and little wound.MethodsSelect 100 patients who have been in the first clinical hospital of china medical university.They are operated on by simple endoscope approach,or endoscopic combined with Caldwell-luc,or lateral rhinotomy approach. The cases are registered and retrospectively analyzed. The information of recurrence is follow-up visited. Every case is graded by Krouse Method, and the recurrence rate of the three methods of different grades is calculated in tabulation.The result is analyzed and discussed with SPSS11.5 statistics software. Thirty-six patiets are operated on by simple endoscope approach. Thirty-five by endoscopic combined with Caldwell-luc,and twenty-nine by lateral rhinotomy approach. The average follow-up time is 3.5 year.ResultsThe total recurrence rate is 20%. The recurrence rate for simple endoscope approach group I is 9.1%, the group II is 9.5%, and the III group is 50%. The recurrence rate for endoscopic combined with Caldwell-luc approach group II is 12.5%, the III group is 12.5%, and the IV group is 33.3%. The recurrence rate for lateral rhinotomy approach group I is 0, the II group is 50%, the III group is 30%, and the IV group is 40%. P<0.05(simple endoscope approach group II and lateral rhinotomy approach group II). There is statistical significance. There are not statistical significance in other groups. Seventy-two patients are for the first time to be operated on, and the recurrence rate is 19.4%. Twenty-eight patients are for the second time to be operated on, and the recurrence rate is 21.4%. P>0.05, There is not statistical significance.Conclusion1. The patients shoule be operated on by simple endoscope approach ,who are in group I or II.2. The patients shoule be operated on by endoscopic combined with Caldwell-luc, who are in group III.3. The patients shoule be operated on by lateral rhinotomy approach, who are in group IV.4. The recurrence cases can be operated on by endoscope approach.
