Object: In this study, we evaluated the tumor necrosis factor-αlevels (TNF-α) and lactate dehydrogenase (LDH)and its iso-enzyme of cerebrospinal fluid (CSF) in patients with meningitis.Material and methods: The concentration of TNF-αwere measured by enzyme-linked immunosorbent assay(ELISA), rate method was used in LDH activity determination and the LDH isoenzyme were determined by agarose gel electrophoresis method in the CSF of 160 patients. Of whom, 53 patients had bacterial meningitis, 58 patients had viral meningitis, and 49 patients had tubercular meningitis. 60 subjects had non-infectious central nervous system (CNS) illness (control group).Result: The CSF TNF-αlevels in meningitis groups were obviously higher compared with the control group (25.64±20.08pg/l, p<0.01). And the concentrations of TNF-αin bacterial meningitis(462.24±369.13 pg/l) was highest. There was no significant difference between viral meningitis(73.47±68.74 pg/l) and tubercular meningitis(62.97±40.58 pg/l p>0.1). The CSF levels of TLDH of bacterial meningitis(201.37±148.06 u/l) were higher than tubercular meningitis (170.32±86.45u/l p<0.01). And tubercular meningitis were higher compared with viral meningitis(79.54±43.71 u/l) and control group(64.56±49.68 u/1). But no significant difference was found between viral meningitis and control group (p>0.1). The levels of LDH4 (22.83±2.08) and LDH5 (35.26±2.18) of bacterial meningitis were higher than the other three groups, the levels of LDH1 (7.84±1.15) and LDH2 (13.01±1.46) were lower than the other three groups (p<0.01); The level of LDH3 (29.48±3.24) of tubercular meningitis was higher compared with LDH3 (16.13±3.95) of the control group (p<0.01); But no significant difference was found between viral meningitis and control group (p>0.1).Conclusion: The cerebrospinal fluid concentration of TNF-α, TLDH and LDH isoenzyme could be helpful in the diagnosis bacterial meningitis,tubercular meningitis and viral meningitis.
