Objective: put into use diffusion tensor imaging and fiber tracking with MRI, compared with the early and later MR imaging manifestation, to sort a quantitative index, provided objective basis for dignosis and assessment of prognosis.Methods: in 96 cases of full term infants and 13 cases of premature infants, it divided into three groups: mild HIE group, moderate HIE group and severe HIE group according as clinical criterion, early time was from 0 to 12 days, late time was from 6 months to 36 months, with MRI and DTI examination, used the functool software of GE to manage DTI, put ROI in T2WI,combined with DEC and FA to fiber tracking,.Results: (1)between MRI and clinical gradation, Kappa value was equal to 0.896 in Kappa test;(2)the more severe the HIE, the lower the MDI and PDI; (3)the same area of FA value was different in different degree of HIE, and it increased by time;(4)while FA value of the corpus callosum splenium was equal to 0.437 or more than, SE was 83.8%,1-SP was 18.6%;while the FA value of the posterior limb of internal capsule was equal to 0.391 or more than ,the SE was 86.5%, 1-SP was 11.9%.Conclusion: (1) there was better coherence between MRI degree and clinical degree ;(2)DTI and FT could provide an quantitative and qualitative measure in the growth of the myelination of white matter; (3) this study put forward the criterion of the early hypoxic-ischemic encephalopathy elementarily (0 to 12 days), FA value of corpus callosum splenium was equal to or more than 0.437, FA value of the posterior limb of internal capsule was equal to or more than 0.391, it could be estimate the prognosis of HIE. While it satisfied the index, there was better prognosis.
