Chapter one:The polycystic ovarian syndrome(PCOS) is a common endocrinemetabolism disorder disease, it affects 5-10% reproductive-aged womenand 75% infertility women of ovulation failure. It is always not onlycharacterized by appearances of androgenemia such as pilosis acne andandroid type fatness, polycystic ovarian, but also representating clinicalmanifestations including oligomenorrhea, amenorrhea or persistencanovulia. In addition, the PCOS also partner has the high risk forwardcomplication formation rate, mainly includes the sugar, the fatnessmetabolism disorder and the cardiovascular disease. Insulin resistanceplay an important role in the development of these diseases. Researcheshave already discovered the risk of insulin resistance, fatness amongPCOS patients is higher than normal. The risk of PCOS concurrently withdiabetes and the cardiovascular disease has the certain difference in thedifferent crowd and the race. At present, there is no correlation researchto characteristic of insulin resistance metabolism in Hans women withPCOS.Objective To investigate the situation of insulin resistance,impaired glucose tolerance and dyslipidemia in the Han nationalitywomen with PCOS.Method Clinical data of 118 patients who suffered from PCOS wereanalyzed. To evaluate insulin resistance and impaired glucose toleranceby oral glucose tolerance test(OGTT) and insulin release test.Simultaneously, dyslipidaemia was estimated including total cholesterol,triglyceride(TG), High-density lipoprotein (HDL), low-densitylipoproteins (LDL), free fatty acid(FFA), apo-lipoprotein A(ApoA) andapo-lipoprotein B(ApoB). Fast blood insulin>15miu/L or glucoseloaded insulin>80miu/L were regarded as insulin resistance. Accordingto this, we divided them into: groupⅠ(IR) and groupⅡ(non-IR). Theage, BMI, waist hip ratio(WHR), and dyslipidaemia between groupⅠandⅡwere compared. Carries on the blood serum examines to T and SHBGin 71. examples, calculate FAI. Analyzed relation between IR and T, SHBG, FAI level.Result 1) The incidence of obesity was 34.75%, android typeobesity was 48.31%. 2) IR and impaired glucose tolerance was 55.08%and 15.25% respectively. Four of them were diabetes(3.39%); 3) 56.78% patients existed dyslipidemia. 4) There were significant differencebetween two groups on BMI, WHR, level of TG, ApoA and HDL. 5) Tlevel does not have difference, SHBG and the FAI level has theremarkable difference between group IR and non-IR.Conclusion Obesity, IR, glucose metabolism, dyslipidemia have ahigh incidence in the Hans women of PCOS. We should pay moreattention to prevention and control of long-turn complication in PCOS.Chapter two:It far confirmed that to PCOS patients specially to those with obesity,insulin resistance examination needs to be carded on as soon as possibleto controlling obese occurrence and further aggravation. Low-densitylipoprotein receptor-related protein—5 has vital and remarkable role toglycometabolism and lipid metabolism. The gene LRP-5 polymorphism isassociated with obesity phenotypes, especially with rs4988300(G/T) locus.But, gene LRP-5 polymorphism is at present no clear how to affectadiposis on the function. Adiposity is also one of common PCOS patientclinical manifestation. However, there are no direct study in relationshipbetween LRP-5 polymorphism and PCOS.Objective To investigate the association between the singlenucleotide polymorphism(SNP) of the gene Low-density lipoprotein receptor-related protein—5 (LRP—5) at rs4988300(G/T) locus andpolycystic ovarian syndrome(PCOS).Methods Denaturing High Performance Liquid Chromatography(DHPLC) was used to amplify the SNP of the gene LRP—5 at rs4988300(G/T) locus for all 110 PCOS patients and the congtrol group(109healthy subjects). Comparisons were conducted in gene polymorphismbetween PCOS patients and congtrol group; fatty group and non-fattygroup, normal glucose torlerance group and inspired glucose torlerancegroup, normal lipid profiles group and altered lipid profiles group inPCOS patients.Results There are 40 unsymmetric figures in all 110 PCOSsamples, 35 unsymmetric figures in all 109 congtrol samples and thers4988300(G/T) locus was confirmed by sampling and sequencing.There are no significant differences both between the PCOS patientsand healthy subjects, but also fatty group and non-fatty group, normallipid profiles group and altered lipid profiles group in PCOS. ConclusionThere are no significant association between the SNP of the gene LRP—5at rs4988300(G/T) locus and glucose and cholesterol metabolism inPCOS. The known number SNP amounts to 700 at present on LRP—5gene, whether other LRP-5 polymorphism lucuses are related with PCOSis not clear.
