Objective: Approach the curative effect and the operating technique of shot-segment pedide screw system (PSs) and posterior lumbar interbody fusi on (PLIF) used in severe isthmic spondylolisthesis.Method: 30 cases of isthmic spondylolisthesis, by the Meyerding standard, the spondylolishesis >33%, and the average was 40%, accompanied with spinal and nerve root compression. Average of patients' Beaujon Functioal Score preoperation was 7.5; the average disc height was 6.2mm. The operating technique were decompression, using shot-segment PSs reduce the spondylolishesis and PLIF with two cages. After operati on the patients nccded to lie on bed for 3 month, antibiotic and neural nutriceutical must be used routinely.Resnlt: The patients were followed up for 14~24 month, the average was 18 month. The incision was primary union, reduce the spondylolisthesis for 91%; average of disc height was 14.9mm, the bony fusion rate was 100% with 4 month for fusion. BFS was 18.5. During follow-up, the disc height and spondylolishesis reduction was not distinctly lost.Conclusion: Shot-segment PSs with PLIF has great advantage to treat the severe isthmic spondylolisthesis, and can retain more lumbar move-segments. The operating technique determined the curative effect. |
