| The ovarian cancer is a kind of common malignant tumor which seriously threatens the women's life and health. The incident rate of ovarian cancer occupies the 3rd place in the gynecologic malignant carcinoma, but which mortality rate occupies the 1st place. Generally, the early diagnosis rate of ovarian cancer is low. 60%-70% patients have had the advanced ovarian cancer when they are diagnosed at last. At present, the method of treatment is cytoreduction and chemotherapy after taking surgery. But finally at least 60% patients obtain the resistance to medicine so that whose (III stage patients') 5 years survival rate only is 20%-30%. Obviously, the chemotherapy does not improve their overall survival rate. The main reason is because the ovarian cancer cells have had the Multidrug Resistance (MDR) which refers to the characteristic of tumor cell which can endure the fatal dose of many kinds of chemotherapy medicine with different structure, function and killing mechanism. Although the reasonable chemotherapy plan may maximum the function of different kinds of medicine, it is considered the defeat because of the appearance of drug resistance. Therefore, the research of drug resistance mechanism in tumor cells, the prevention of drug resistance, the conquer of drug resistance and the attempt of reversing drug resistance have become the hot spot and the difficult problem in treating ovarian cancer as well as one of the most important targets of enhancing the treatment effect and improving survival quality of ovarian cancer patient.The multidrug resistance in tumor is affected by multiple factors. At present, the research in resistance mechanism of ovarian cancer cells has focused in the following aspects:①The research of genes and its expression related with drug resistance.②The research of enzymes related with drug resistance.③The research of other ingredient related with drug resistance. eg. GSH and MT.At present, the primary methods of treatment for the patients with drug resistance ovarian carcinoma are surgery and chemotherapy. At least 60% of advanced ovarian cancer (III stage, IV stage) will recur after they obtained when they complete the surgery and the chemotherapy. There is dispute in surgery index, surgery opportunity and the relationship between chemotherapy and ovarian cancer patients who resist chemomedicine. We should select the suitable surgery method and the surgery scope according to the patient different situation. Usually, it is thought that the se(?)ondary cytoreduction to the ovarian cancer patients who resist chemomedicine is only suitable for following several kinds of situations.①Recurring tumor stove is explicit and relative isolation, and is estimated to be excised probably.②Patients have a better response to the former chemotherapy and has the more sensitive chemotherapy plan which may use after the surgery.③In order to relieve symptoms and so on intestinal obstruction. Simultaneously we also must unify patient's life condition and the economical situation to appraise the patient possibly degree of endure to the surgery. It should be confirmed that patients may profit from the surgery. Chemotherapy has two main aspects: First, choose the chemomedicine or chemotherapy plan without cross resistance with initial chemotherapy. VP-16, PLD, Caelyx, Topotecan Gemcitabine and Oxaliplatin all have the very good curative effect. Second, attempt to use reversal medicine. Including①calcium passage blocker;②The inhibitor to the protein adjusting calcium;③Immunity conditioner;④Chinidine kind;⑤Hormone, hormone antagonist compound. Moreover, large dose chemotherapy under the support of circumference hemopoiesis stem cell transplant also is one kind of new attempt.The ovarian cancer is one kind of seriously disease which threaten women' survives. Along with the widespread application of some new chemotherapy medicine, some new progress in researching chemomedicine resistance has been appeared in recent years:①The model in cultivating ovarian cancer cells resistant to chemomedicine has changed from monolayer model to three-dimensional model.②Researchers have begun to consider the interstitial influence outside cells to discusses the influence that cell micro environment effect on the sensitivity to chemotherapy in tumor cells.③At present, there is evidence to indicate the close correlation between tumor shift and the resistance to chemotherapy. Therefore, the syntheses in the research of medicine resistance and other biologic characteristic of tumor cells will be on focus.
