AIM: To study the expression of p53 and MMP-7 in chordoma and analyze thecorrelationship between pathological characteristic and clinical behavior of chordoma.To explore the role of p53 and MMP-7 in pathogeny, development and prognosis inthe patients with chordoma in order to provide scientific base for the diagnosis,treatment and estimate prognosis ofchordoma.METHODS: 23 chordoma cases were available from Taian centeral hospital,affiliated hospital of Taishan medical collegeand Number 2 hospital of Taian.Histologic sections were stained routinely with H&E for diagnostic purposes. Thefollowing studies were conducted using sections from 10% formalin-fixed, paraffinembedded tissue samples, highlighting the representative areas of the tumor. Theexpression of p53 and MMP-2 in chordomas was studied by immunohistochemistrystaining. The sections stained p53 and MMP-7 were graded according to the ratio ofpositive cells and staining intensity. Follow-up information of recurrence and survivaltime were available for 23 patients. The data were analyzed with SPSS for windows10.0.RESULTS: In 23 chordoma cases, 14 cases were mainly composed ofphysaliferouslike cell in which 3 cases recurred and 9 cases were mainly composed ofastral cells in which 8 cases recurred. There were significantly differences in therecrudescent ratio between the two types of chordoma(P<0.01). 16 cases expressedp53, and positive ratio was 47.1%;9 cases were in primary groups and positive ratiowas 39.1%; 7 cases were in the recurrence group and positive ratio was 63.6%.Theexpression of p53 in the recurrence group was significantly higher than that of in theprimary group (P<0.01). 22 cases expressed MMP-7, in which 12 cases in primarygroups, 10 cases in the recurrence group. The positive ratio of MMP-7 was 52.2% inprimary groups and 90.1% in recurrence group. The expression of MMP-7 in therecurrence group was significantly higher than that of in the primarygroup(P<0.05);But there was no significantly difference in the expression of p53 and MMP-7 between men and women patients as well as between younger and eldepatients.The average survival time of patients with positive p53 expression was 35.14months, and 72.25 for the patients with negative p53 expression(P<0.01). The averagesurvival time of patients with positive MMP-7 expression was 78.64 months, and86.33 for the patients with negative MMP-7 expression(P>0.05). The average survivaltime of patients with both positive p53 and MMP-7 expression was 21.69 months.There was significant difference in survival time between the patients with twopositive expression and only one positive expression (P<0.01).CONCLUSION: Chordoma which was mainly composed of physaliferouslikecells and had more pituita in mesenchyma had better histological differentiationgrade,but chordoma which is mainly composed of astral cells had higher recurrencerate than mainly composed of physaliferouslike cells.The expression of p53 inrecurrent group was significantly higher than no-recurrent group;The recurrent timeafter operation of patients with positive p53 expression was earlier than thepatients with negative p53 expression,and the survival time of the former wassignificantly shorter than the latter:The expression of MMP-7 in recurrent group wassignificantly higher than no-recurrent group and primary groups; The expression ofMMP-7 in tumour tissue was significantly higher than tumour acroteric normal tissue;The recurrent time after operation of patients with positive MMP-7 expression wasearlier than the patients with negative MMP-7 expression,but there were nosignificant differences in survival time;The patients with conexpression of p53 andMMP-7 recurred in the earlyier stage and the survival time was shorten obviously.
