| Objective It has been aroused intensive attention on the glucose metabolismsituation of coronary heart disease (CHD) patients. Europ Heart Survey hasproved that over 2/3 CHD patients also has abnormal glucose metabolism. What'smore, about 2/3 patients with acute myocardial infarction have abnormal glucosemetabolism by nature but not only caused by transitory high glucose duing tostress response. Previously, most of investigations were based on theWhites. Due to the differences betweent areas and races, it is deserved to probethat if there will be the same conclution on CHD patients in China. The purposeof this essay is to investigate the glucometablic changes and TCM-syndromechanges of patients with CHD and CHD combined with abnormal glucose metabolism.Methods All inpatients with CHD in cardiology department of The FirstAffiliated hospital of Guangzhou University of Chinese Medicine wereconsecutively recruited from Jun, 1, 2005 to May. 31,2006. A total 134 patientswith CHD were included in the study. The transformation of the glucose, theclinical-affair frequency and the typing of syndromes of TCM were observedafter one year by uniform standards. Results The prevalences of patients withDM and impaired glucose regulation were 28.3ï¼…and 10.9ï¼…with CHDrespectively. The total prevalence of abnormal glucose metabolism andclinical affairs were 39.2ï¼…and 26.5ï¼…. The prevalences of patients withDM in CHD combined with impaired glucose regulation was 16.7ï¼…. Theclinical-affair frequency was 38.5ï¼…. The clinical-affair frequency of DM inCHD was 66.1ï¼…. The typing of syndromes of CHD patients were mainly Qi deficiencywith blood stasis and turbidphlegm. While the patients with CHD combined withabnormal glucose metabolism were mainly Qi and Yin deficiency with blood stasisand turbidphlegm. Conclusion The frequecy of disturbed glucose metabolism is considerably high in patients with CHD. Also the clinical-affair frequencyof abnormal glucose metabolism was much higher than that of normal glucosemetabolism. Without the use of oral glucose tolerance test (OGTT),a majorityof patients with abnormal glucose metabolism would have remained undiagnosed.In total, 80ï¼…impaired glucose regulation and 62.3ï¼…DM would remainedundiagnosed by fasting plasma glucose (FPG) detection alone. The therapeuticprinciple in CHD is supplying Qi, eliminating phlegnand activating bloodcirculation while those with abnormal glucose metabolism should lay stresson nourishing Yin. |
