Objective The purposes of the investigation were to determine the prevalence of birth defects and to examine the risk factors that maybe make this population more susceptible to the development of birth defects and to supply some key risk factors for the intervention project on birth defects in Gansu province, West China. The pregnant woman childbirth present situation to discusses adverse pregnancy outcomes the formation rate, to analysis adverse pregnancy outcomes correlation dangerous factor, to provide the basis for optimum methods of child-bearing and child-rearing the thorough research.Method [1] A baseline survey on birth defects: Forty communities of four counties were randomly selected by stratification cluster sampling for the survey on the states of economic and geographical features in the province. These infants born and stillbirth from January 1 of 2001 to December 31 of 2001 were firstly investigated in the department of gynaecology and obstetrics, Service station of birth control, Service station of woman and children, county and communities hospitals. These disease types of birth defects were classed by the diagnosis standardization of ICD-10[2] The case-control study on birth defects: Forty communities of four counties were randomly selected by stratification cluster sampling for the survey on the states of economic and geographical features in the province. The case-control study was adopt in which a matches including both a case of infant of birth defects and control of two normal infants who had same age, sex and inhabitation. Infants born under five years old were investigated. The parents of these infants were contacted and completed by investigators regarding the prenatal and postnatal history of their child. The total sample was both case group 295 and control group 590.[3] Adverse pregnancy outcomes: Using two provincial levels hospitals and a provincial level faculty hospital were from January, 2004 obstetrics was hospitalized to December, 2005. The pregnant woman medical record total 5,002 extracts correlations information, and carries on the analysis to the correlation factors.Result [1] All birth defects 16.35‰,Birth defects rate in 18.55‰male, in 13.9‰female. Neural tube defect 7.07‰, low weight birth 4.98‰, limb defects 2.09‰, cleft lip and palate 0.80‰and Down's syndrome 0.48‰. These incidence rates of birth defects in four counties in Gansu: Weiyuan county was 23.51‰, Huei county was 22.71‰, Duenhuang county was 7.81‰, Jingchuan county was 15.22‰. The mortality of birth defects was 7.69‰, the mortality of birth defects attributable to IMBD was 48.98%.[2] The risk factors of birth defects in Gansu were: history of contacting with pesticide in forepart pregnant, OR was 7.73, OR95%C.I was 3.55-16.81; dose history in one mouth before pregnant, OR was 7.34, OR95%C.I was 3.96-13.62; history of contacting with pesticide before pregnant, OR was 7.11, OR95%C.I was 3.37-14.98; infected history in forepart pregnant, OR was 4.38, OR95%C.I was 3.03-6.64; miscarriage history, OR was 3.21, OR95%C.I was 1.71-6.03; smoking history in forepart pregnant, OR was 2.31, OR95%C.I was 1.75-4.47; dose history in forepart pregnant, OR was 2.28, OR95%C.I was 1.31-2.85, history delivery infants of birth defects, OR was 1.71, OR95%C.I was 1.10-2.67.[3] Formation rate of adverse pregnancy outcomes was 19.63%, every kind of adverse pregnancy outcomes formation rate was: natural abortion (10.58%), premature delivery (2.70%), embryo dies in the palace (1.58%), embryo poverty-stricken (1.54%), fetal macrosomia (1.42%), birth defects (1.04%), hydatidiform mole (0.79%). Pregnant woman's age, the occupation, the unusual pregnancy history, and namely toward produce time with namely toward embryo were these dangerous factor which adverse pregnancy outcomes occurs.[4] The natural abortion rate was 10.58% of the in pregnant woman hospitalized, accounting for the proportion in every kind of natural abortion type by the retained miscarriage was biggest, minister 41.21%; These causes to the natural abortion factor according its relative danger in turn were formerly the natural abortion history, formerly unusual pregnancy history, the advanced age, formerly the embryo, formerly the induced abortion history, formerly produces the order, and the occupation.Conclusion The incidence rate of birth defects in Gansu was one of the highest area in China. The risk factors of birth defects in Gansu different area were not only same factors but also different factors. The risk factors of birth defects in Gansu different area were: history of contacting with pesticide in forepart pregnant, dose history in one mouth before pregnant, history of contacting with pesticide before pregnant, infected history in forepart pregnant, miscarriage history, smoking history in forepart pregnant and dose history in forepart pregnant and history delivery infants of birth defects. The formation rate adverse pregnancy outcomes was higher, these local the actual situation should increase optimum methods of child-bearing and child-rearing the propaganda dynamics, at the same time develops has the pointed synthesis intervention, to reduces adverse pregnancy outcomes the formation rate.
