To assess left ventricular diastolic and systolic function in hypertension patients with different LV geometry using stain imaging and stain rate imaging. Research objects were 88 cases of hypertension and 35 normal control subjects. Using conventional two-dimension and M-mode echocardiography, the size of left atrium and left ventricular, pulsed Doppler echocardiography E,A and E/A, left ventricular ejection fraction (EF),cardiac index(CI), fractional shortening (FS),stroke volume (SV),left ventricular mass index(LVMI)and relative wall thickness (RWT) were measured. Taking tissue Doppler imaging of apical 2-chamber view(inferior,anterior),apical 3-chamber view(anterior septum,posterior),apical 4-chamber view(posterior septum,lateral).Take Strain imaging and Strain Rate imaging .Analysis strain and strain rate curves of all walls in basal and middle segments of LV in diastolic earlier stage,diastolic advanced stage and systolic stage. According to LVMI and RWT, hypertension patients divided two groups–left ventricular normal configuration (LVN) and left ventricular reconstitution (LVA). Analyse change of strain and strain rate in different myocard segment in normal control group,LVN and LVA. Result display: 1. ultrasound cardiogram basic index:(1)Left atrial diameters of LVN and LVA were a significant increase than normal control group. (2)Comparing with healthy subjects, LVMI and RWT were a significant increase. (3)Pulsed Doppler echocardiography : comparing healthy subjects, E and E/A decreased significantly. A increased significantly. Comparing LVA with LVN, E and A decreased significantly, and A had not changed obviously. 2. SRI index: (1) Diastolic function: Except basic segments of myocardium in inferior, SR all decreased in diastolic earlier stage in LVN and LVA. SR of LVN basic segment in posterior septum and LVA middle segment in posterior septum and posterior and basic segment in inferior and anterior in diastolic advanced stage increased significantly.(2)systolic function:LVN SR of middle segment and basic segment in inferior were lower than normal controls significantly,but LVA SR of inferior,lateral,anterior septum were lower than normal controls significantly.Strain imaging and strain rate imaging provide a sensitive, convenient and reliable index in evaluating cardiac function of hypertension patients; are a no-insult technique.
