Objective: The aim of this study was to investigate the clinical manifestation,diagnosis and treatment of Histoplasmosis. Methods: A retrospective study was performed of 3 cases with Histoplasmosisadmitted into our hospital from May 2003 to December 2006. Result: (1) Clinical manifestation: Symptoms of Histoplasmosis include fever(2cases), weight loss (2 cases), adenophyma (1 case), cough (1 case),night sweat (1 case). Signs of Histoplasmosis include erythra (1 case),splenomegaly (1 case), hepatomegaly (1 case).(2) Supplemental examination: The diagnosis was established by lymphonode biopsy in 2 cases, by CT-guided percutaneous aspiration lung biopsy in 1 case. A bone marrow sample from 1 patient had a positive fungal stain.(3) Treatment: Liposomal amphotericin B combined with Itraconazole, amphotericin B combined with Itraconazole, and Itraconazole alone, were used for the treatment of Histoplasmosis, respectively. All patients were cured in the current study.Conclusion: The clinical manifestations of histoplasmosis were atypical. Culture is the gold standard for diagnosis of histoplasmosis, however, it is fairly time consuming. By contrast, fungal staining is rapid and easy to practice, although it's sensitivity is lower than that of culture and antigen detection. Repeated examination of tissue smear or biopsy sample can increase the detection rate. Liposomal amphotericin B is generally well tolerated, may be indicated for patients who cannot tolerate amphotericin B.
