Relationship Between The Plasma Homocysteine And Peripheral Neuropathy Of Type 2 Diabetes

Objective To study the change in plasma homocysteine(Hcy) after methionine loading test(MLT) and its significance in patients with peripheral neuropathy of type 2 diabetes(DPN).Methods 37 DPN patients,32 type 2 diabetes patients with no complication(NDC) and 29 normal controls(CON) received MLT test.The plasma fasting homocysteine(Fhcy) level and the Hcy level 4 hours after MLT test(Phcy) of these subjects were measured by fluorescence polarization immunoassay(FPIA) before and 4 hours after PML test.The levels of fasting plasma folate and vitamin B12(VitB12) were tested by radioimmunoassay. FBG,PBG,Cr,BUN,TC,TG and HbA1c were also detected.Results The levels of FBG ,PBG and HbAlc in NDC and DPN group were higher than those in the control group(P<0.05).The Fhcy levels were 10.40±1.91 μmol/L and 10.90±1.92 μ mol/L in DPN and NDC group respectively,both higher than that in control group(8.48±1.34μ mol/L,both p<0.05).The Phcy in DPN group (24.73 ±1.43 μ mol/L) was higher than that in the control group(18.62±1.35 μ mol/L, p<0.05). Both Fhcy and Phcy levels in DPN group had no significant difference compared to NDC group. Fhcy was not related to folate,VitB12,age and sex in total diabetes group.The folate and VitB12 levels in both DPN and NDC group had no significant difference ,compared to the control group.The number of hyperhomocysteinemia (Hhcy) patients diagnosed by the level of Fhcy was 10 and it increased to 23 diagnosed by the level of Fhcy and Phcy after PML test.Conclusions Hhcy is not an independent risk factor of DPN.Hcy is not related to folate and Vit B12, and the folate and VitB12 levels are not low in both DPN and NDCpatients. MLT test maybe helpful to identify more potential Hhcy patients in DPN and NDC patients.
