The Experiment Of Dihuangjiangya Decoction Effecting In The SBP, The Ang Ⅱ And Ald In Blood Sample Of Renovascular Hypertension Rats

Kidney are closed interrelated to hypertension . Seen from the angle of occurrence mechanism ,kidney participates in every kind of hypertension and even is the main playing factor, for example the effect of renin-angiotensin aldosterone system on action of adjusting blood pressure in essential hypertension. Hypertension caused by renal parenchymal or(and) renal vascular disease is designated by the name of renal hypertension.It is divided into renal parenchymal hypertension and renovascular hypertension according to anatomic site.On the other hand It is divided into volume pendence and renin pendence according to occurrence mechanism. Renal hypertension is majority of secondary hypertensive pathogenesis, about 70%. The incidence rate of different renal diseases complicated by hypertention were as follows: glomerular diseases 85%, tubular interstitial diseasea about 63%, serious renal failure is above 90% . Renovascular hypertension is caused chiefly by renal artery lesion ,and the usual causations include aortitis(64%),fibromuscular dysplasia(15%)and arteriosclerosis etc that cause narrow of the renal artery orifice or main truck or the main artery branch, etc. Renal parenchymal hypertension is caused chiefly by various acute or chronic glomerulo nephritis,diabetic nephropathy,chronic pyelonephritis,connective tissue disease,multicystic kidney , etc that is renal parenchyma pathologic change .Renal disease could cause hyperten- sion ,conversely , hypertension could accelerate the developmentof renal disease and promote renal function to worsen .So it is very important to control the blood pressure at the same time treating the protopathy .Chinese prophylaxis and treatment enchiridion of hypertension indicte: renal disease(including diabetic nephropathy) should control blood pressure strictly (<130/80mmHg) .When the urinary protein >1g/d, the objective of blood pressure <125/75mmHg and playing the urinary protein down to be normal in so far as possible .Antihypertensive drug in western medicine avail quickly against renal hypertension and could protect the kidney to some extend.It is the preferred medicine that treat medium hypertension and even serious one.But the shortcoming that side effects is large and the charge of blood pressure is rapid is tough thing in clinical therapertics. But herb play the hypertensive activity steady through regulating qi-blood and balaming yin-yang .Herb may ease accessory symptom;protect target organ;delay renal failure .And the side effects of it is little.for the elderly patient that is ill with angiosclerosis herb could avoid large fluctuation better than western medicine.So herb is suitable for taking long.In this experiment Two-kidney1-Clip(2K1C) rats are used to reestablish the model of renovascular hypertension .40 rats were randomly divided into five groups : sham group , 2K1C group , drug high dose group(80g·kg- 1), drug low dose group(40g·kg- 1), captopril guoup(28mg·kg- 1).the number of every group rats is 8 .Every rat were given 1 irritate stomach every day during therapy. The drug dose that every rat was given one day is as follows:dihuangjiangya decoction 80g·kg^-1 in drug high dose group; dihuangjiangya decoction 40g·kg^-1; captopril guoup 28mg·kg^-1; 2ml physiological saline sham group and 2K1C group .The volume of drug given is 2ml/100g.this dose is used in according to reduction formula of weight and dose between human and animal written by chenqi.The drug dose of dihuangjiang decoction is equal to 21 times and 11 times than clinical one for person with 60Kg weight every day. The therapy had lasted 3 weeks.every rats was taken systolic blood pressure(SBP) three times before and after therapy respectively .And the three times SBP was averaged.after 3 weeks for therapy rats were sampled blood on eyeball.We detect the content of angiotensinⅡ(AngⅡ) and aldosterone(Ald) in blood sample .The SBP, AngⅡand Ald were compared .Results:①Compared with 2K1C group and prior to therapy ,the SBP of rats were decreased remarkablely in each treated group after therapy(P<0.01).②The contrast of effects in decreasing blood pressure is not remarkable among drug high dose group , drug low dose group and captopril guoup(P>0.05).③Compared with 2K1C group and prior to therapy , the content of angiotensinⅡand aldosterone were decreased remarkablely in each treated group after therapy(p<0.05,p<0.01).④The contrast of effects in decreasing the content of angiotensinⅡand aldosterone is not remarkable among drug high dose group , drug low dose group and captopril guoup(P>0.05).Conclusion: dihuangjiangya decoction could decreased remarkablely the SBP, AngⅡand Ald content of 2K1C renovascular hypertension rats .And the contrast of the effects isindistinctive between dihuangjiangya decoction and western medicine matched group .Decreasing the plasma RAAS probably one of mechanism of dihuangjiangya decoction decreasing blood pressure. Uncaria ,Fructus lycii,spatholobus stem and achyranthes have different inhibition on angiotensin converting enzyme in .all of them could reduce the content of AngⅡ.The uncarine in uncaria could inhibite the liveness of sympathetic nerve directly or indirectly ; make the retardarce of receptor of sympathetic nerve in extraglomerular ceel;inhibite renin from secreting.At the same time the effect that renin-angiotensin stimulates aldosterone secretion is abated .And the content of Ald is decreased. Poria ,Oriental water plantain rhizome , Radix-polygoni multiflori ,Spica prunellae , Rhizoma chuanxiong decrease the blood pressure through the mechanism of diuresis , reducing the cardiac rate ,expanding the veins ,decreasing the vascular resistant,etc.In the side of traditional chinese medicine , renal hypertension has its origins in congenital constitution ,dietary overstrain ,spirit stimulating ,etc .These factors lead to deficiency of liver Yin and kidney Yin . Due to water failing to nourish wood and failure of Yin to keep Yang ,live Yang hyper-act and weaken parietal upper or reservoir of marrow is insufficient and qi-blood could not supply brain plentifully .So the vertigo and headache has happened .When the course of disease is long , deficiency of Yin affects Qi and both Qi and Yin systems are deficient. Qi as commander of blood , deficiency of Qi could notfurther the movement of blood .So the blood stasis produces .At the stage of renal failure spleen and kidney are asthenic , Yin- Yang- Qi-Blood are all deficient, Qi does not transform into water , Yang does not transform into turbid urine. Demp and turbid urine are retentive. Phlegm and blood stasis accumulate.Body resistance has been weakened while pathogenic factors prevailing .And there is struggle between vital energy and pathogenic factor. Demp and turbid urine attack upper,even occur convulsionary wind .A combination of factors above leads to make hypertension be more serious gradually. The main treatment are to nourish liver and kidney ;to calm liver to suppress hyperactivity of liver yang .the assorted treatment is to promote blood circulation and remove blood stasis; to remove stagnation of sputum and demp.The compatibility of dihuangjiangya decoction is based on analysis above and thus these decoction play a role of decreasing blood pressure .
