| As dramatic increase in the number of diabetic patients in the world, poor glycemic control and the increase in the microvascular and macrovascular complications would seriously threaten the health and quality of life of diabetic patients. However, in addition to the tight blood glucose control, reducing blood glucose smoothly and the incidence of hypoglycemia has become the goal of treatment. Short-acting insulin with slow onset and peak time should be injected 30 minutes before meals and can lead to blood sugar fluctuations and hypoglycemia. Insulin analogues developed by the gene recombination technology with changes in the amino acid structure has faster onset and peak time and can be injected before meals or at meals. It also has short duration time and is in accordance with physiological postprandial insulin secretion curves. To investigate the effects of insulin analogues and the incidence of hypoglycemia, diabetic patients with short-acting insulin analogues,regular insulin and premixed insulin were retrospectively analysed.The results show that insulin analogues with insulin pump therapy for one week, fasting glucose and postprandial glucose have dropped significantly, as compared with those before treatmen, which was statistically significant (P<0.001). The 2-hour postprandial glucose after breakfast is lower than the regular insulin. The successrate of blood glucose control in patients with insulin analogues is higher than regular insulin and premixed-insulin. The time of blood glucose for achieving the target in patients with insulin analogues is shorter than regular insulin and premixed-insulin. With no significant difference, the dose of insulin analoguses is less and the incidence of hypoglycemia is lower than regular insulin and premixed-insulin. But the event of hypoglycemia is lower with significant difference.We find that insulin analogues can effectively reduce postprandial hyperglycemia. The time of blood glucose for achieving the target is shorter and the success rate of blood glucose control is higher in insulin analogues patients. The action of insulin analogues with lower incidence of hypoglycemia and less dose is in accordance with physiological insulin secretion curves. The side effects will be analysed with more patients and long time except hypoglycemia. |
