The infections of mycoplasma or chlamydia are closely related with a variety of clinical diseases. Reproductive tract infections caused by mycoplasma and Chlamydia have exceed gonorrhea ,become one of the highest incidence rates of sexually transmitted diseases. The infections of mycoplasma or chlamydia can cause non-gonococcal urethritis and cervicitis, pelvic inflammatory disease and other gynecological inflammation, women can lead to infertility, ectopic pregnancy and various adverse pregnancy outcomes, even neonatal can be involvemented through the fetal placental infections. In addition, some studies have shown that the infection of chlamydia trachomatis are potential risk factors for cervical squamous cell carcinoma. Therefore, actively exploring the effective treatment of the disease caused by chlamydia, mycoplasma infection is of great clinical value and practical significance.The treatment of chlamydia and mycoplasma infection has conducted extensive research, while with the emergence of drug-resistant bacteria and the change of type, the curative effect of traditional therapy have declined gradually, so some patients appear recurrent or persistent positive creating tremendous burden of physical and mental. And using tetracycline and erythromycin for a long time can bring serious gastrointestinal reactions, may cause certain liver damage. Therefore, more effective treatment should be researched to reduce physical and psychological hurt of the patients and improve the cure well rate. The purpose of this study is to cure female with the genitalia infections of mycoplasma or Chlamydia. The study mainly takes the therapy of local application to the cervix and vagina to the patients. It determines the prognosisand efficacy and explores the clinical efficacy and feasibility of the treatment in order to providing a more rapidly and effectivly clinical treatment that can be used widely in the future.We choose 168 outpatients who infect Ureaplasma urealyticum, Chlamydia trachomatis in genital tract. All patients checked vaginal secretion before the treatment, and the results showed that Neisseria gonorrhoeae was negative. If they infected trichomoniasis, candidiasis, the patients must be cured well. All patients are divided into four groups according to methods of treatment. Group A: 51 patients were treated with systemic administration. Twenty of 51 patients were treated with azithromycin by orally administration, 0.5g twice daily for 7 days. Eighteen of 51 patients were treated by intravenous administration for 7 days. Both orally and intravenous administration are 13 patients; Group B: 52 patients took local application to the cervix and vagina once daily for 3-5 days; Group C: 53 patients not only took orally but also took local application to the cervix and vagina. The patients with pelvic inflammatory disease used Chinese traditional medicine at the same time; Group D: 12 patient refused any therapy, only were observed.The results show the cure rate of the three group is 62.75%, 80.77%, 92.45%respectively. There is distinct difference between group A and group B(P<0.05). There is distinct difference between group A and group C(P<0.01). No distinct difference between group B and group C(P>0.05). The results suggest that local treatment not only can kill the pathogen in reproductive tract thoroughly and specifically but also can clear up the cervicovaginal secretion , make the vaginal environment better, and reduce the breeding of the pathogen, thereby improve efficacy significantlyTo the patients not treated, there is no distinct difference among the three groups on the curative effect(P>0.05),but when the patients are rechecked after stopped treating for three month, it can be found that relapse rate of group A is higher than group B(P<0.05)and group A higher than group C significantly(P<0.01). To the patients treated , the curative effect of group B is better than group A(P<0.05), and group C is better than group A significantly(P<0.01). The results show again that local treatment can kill the pathogen in reproductive tract thoroughly and specifically. And compared the patients among the group B, it can be found that there are no distinct difference(P>0.05)of the curative effect whether the patients were treated or not. The local treatment can achieve satisfactory results to recurring patients or persistent positive patients.To the patients with symptoms, those with abnormal secretion, vulva pruritus, cervicitis and cervical erosion can get good curative effects in group B and group C, those with symptoms of urinary system infection get worse curative effects in group B. It indicates local application is a specific therapy to the cervicovaginal infections, but it has no evident curative effects in the body else alone. The patients with urinary system infection have good curative effects in group A and group C, and group C are better than group A. It illustrates local application to the cervix is hard to chang the symptoms of urinary system ,but it can get some auxiliary effects.The use of systemic administrations combine with local application can make pathogen die out more quickly, and can achieve synergistic effect, while the adverse reaction of the medicine is not increased. To the patients had pelvic inflammatory disease ,the curative effects in group A and group B are both not very well, there are no distinct difference(P>0.05).While the patients take Chinese traditional medicine by orally canget Satisfactory results .The above consults illustrate local application can get evident curative effects to the patients with the symptoms of gential tract, and those with complications else take orally medicines at the same time can also get satisfactory curative effects.The side effect of group A is higher than group C (P<0.05), and group B haven't evident side effects. It indicates local application to gential tract can kill the pathogen of cervix and vagina thoroughly, release the complaints of patients most generally and make the lowest side effects.The patients of untreated observational group don't turn into negative when they recheck, and some patients have appeared clinical symptoms. So the patients of the gential tract infected by CT and UU have little chance to turn into negative ,they should be cured promptly in order to get best curative effect. But we must attention the means of therapy to avoid causing great burden of patients in mind or costs because of excessive treatment.In conclusion, we consider the local application therapy of inflammation of gential tract infected by CT or UU is better than the traditional therapy. It can relief the body and mind of patients very effectly and the patients are easy to achieve the therapy. If the patients can not be cured well for a long time or have some complications, they can take orally medicine at the same time,and can get the satisfactory curative effect. So it can be generalized very extensively.The last conclusion of this reasearch is below:1. The main therapy of local application to cervix and vagina has a lot of merits , such as good curative effect,short course of treatment, low relapse rate and minor cost,and so on. It has evident availability and confidence, and the patients are easy to achieve ,so it is a shortcut and high-performance therapy.2. local application has little side effects, and it can alleviate the sufferings of patients evidently after the patients apply medicine.3. local apply antibiotic to cervix as a new therapy of the infection of CT and UU is better than the traditional therapy . It can relieve the fear and mortgage of the patients because of applying medicine again and again.
