[Objectives]This study was aimed at learning the leukemic patients' clinical features, laboratory test features to induct clinical work.[Methods]221 cases of patients who were diagnosed as leukemia from July 2004 to December 2005 and accepted their first treatment in Qilu Hospital are studied. Of the 221 cases, 125 are male, 96 are female. The age ranges from 8 to 81. The median age is 40. The number of acute leukemia is 182, the number of chronic leukemia is 37, and there were additional 2 cases transferred from NHL(non-Hodgekin lymphoma). The cases were diagnosed if the leukemic cell in marrow aspirate equals to or exceeds 20%. The subtypes were confirmed according to the FAB standard. The induction regimen of ALL ( acute lymphocytic leukemia) are mainly VDLP and DOCP. The induction regimens of APL(acute promyelocytic leukemia) include one, two or three of ATRA(all-trans retinoic acid), DA(daunorubicin plus Ara-C) and arsenic trioxide. The induction regimen of the other subtypes of AML are mainly DA and MA(mitoxentrone plus Ara-C). The patients of CML in chronic phase were treated with hydroxyurea and interferon, the patients in accelerated phase and blast crisis were treated with chemotherapy. When the Hb decreased to the level lower than 80g/L, the patients would be given RBC transfusion. When the platelet count decreased to the level lower than 20 × 109/L or the patient had bleeding symptoms, the patient would be given platelet transfusion. When the neutrophils' count of a patient of ALL decreased to the level lower than 0.5 × 109/L or the temperature exceeded 38.5 ℃ and the fever had maitained for more than 2 days, the patient would be given subcutaneous G-CSF, and the G-CSF would be stopped as soon as the counts of WBC and neutrophil and the temperature had settled down to normal. Antibiotics were given when there was sign of infection. The effect of the induction therapy is evaluated according to the standard made by the Nation-wide Leukemia Chemotherapy Meeting held in 1987. Early death is defenited as death within 30 days after diagnosis or death within 14 days after the beginning of induction therapy. The information were recorded with uniform formats. The datas were dealt with Excel and SPSS. [ Results and conclusions ]1.ALL patients' complaint is mainly fever, fatigaue and bleeding. L3 type is seldom. Pre-B type predomitates in ALL. The anemia before treatment is mainly intermediate anemia. Induction regimen is mainly VDLP and CR is 62.5%. The amenia after induction is mainly intermediate anemia. After induction, most patiens's lowest level of peripheral blood neutrophils is 0.1-0.5×109/L2.AML patients' complaint is mainly fever, fatigue and bleeding. The anemia before treatment is usually intermediate anemia. M5 is the most in the 8 subtypes followed by M3. ATRA+DA+arsenic trioxide is used most in M3. DA predominates in the induction regimens for the other 7 subtypes. The anemia after treatment is usually intermediate anemia. The number of patients whose lowest level of peripheral blood neutrophils is 0.1-0.5×109/L is most.3.CML patients' complaint is mainly fatigue, distention of abdomen and anorexia. Anemia before treatment is usually mild or intermediate. The peripheral blood WBC count ranged from 8.9 to 437. 2×109L, and the number of patients whose WBC count ranged from 50 to 200 × 109/L is most. The platelet count in peripheral blood ranged from 24 to 1615 × 109/L, and the number of patients whose platelet count exceeded 450×109/L is most. After treatment most patients suffered from mild or intermediate anemia, and the WBC and platelet count reached to normal.4.Number of CLL cases is small. The clinical manifestations are various. Some patients without symptoms went to doctor for the abnormal pripheral blood count. Chlorambucil and cyclophosphamide were use most.5.For AML patients , the level of Hb and Plt in peripheral blood after first treatment is lower than the level before the first treatment.
