Objective: To explore A new surgical procedure for phallic reconstruction. Methods: Four patients with penile loss were reconstructed by transferring a deep inferior epigastric perforator (DIEP) flap with implanting the 12th costal cartilage. Results: Since December 2003, we report our technique using a deep inferior epigastric perforator (DIEP) flap as the donor tissue in four patients. All of the flaps survived successfully. With the follow-ups from five months to three years, the appearance of the neopenis and donor-site scar were satisfactory. No one developed fistula formation or urethral stenosis. All patients were reported the return of erogenous sensibility of the neopenis leading to orgasm and achieving penetration during sexual intercourse. Conclusion:This new procedure, designed reasonable, which can be completed in one-stage, provides an esthetically acceptable and functional neophallus. It seems to be a useful alternative in penile reconstruction.
