Management And Outcomes Of Prostate Cancer: A Report Of 238 Cases

ObjectivesTo summarize the diagnostic and therapeutic experience of prostate cancer (Pca), and analyze the oncologic outcome of treated by different means of Pca in our institution of Zhejiang province, in China.Patients and Methods238 consecutive patients were reviewed retrospectively with a mean age of 72 ± 7 years between February 2000 and Apirl 2005. Tumor characterize, clinical features , management and oncologic outcomes were analyzed in this study.Results134(56.3%) and 130(54.6%) patients had abnormal digital rectal examination(DRE) and ultrasonography(US), respectively. 189(78.3%) patients had pretreatment serum prostate-specific antigen (PSA) level more than 10 ng/ml. Gleason score was evaluated in only 71 patients (29. 8%). 43 patients with localized Pca received radical prostatectomy (RP). A total of 175 patients underwent bilateral orchiectomy, of whom, 82 received suprapubic prostatectomy additionally. Only prostatectomy or transurethral resection of prostate(TURP) was performed in 20 patients(17 plus 3) for clinical incidental prostate microcarcinoma. The mean followup was 34 ± 17. 7 months (range 6 to 76) of 211(88. 7%) patients. 34(14. 7%) were dead 6 to 53 months (mean 25) after therapy because of Pca metastesis, 3 of whom died of severe pneumonia, lung cancer and cerebrovascular accident, respectively.Conclusions1. The combination use of DRE, serum PSA measurement and ultrasonography can improve the sensitivity of prostate cancer.2. Radical prostatectomy had grate survival for localized prostate cancer, while hormonal therapy was the mainstay for non-localized cancer.3. Palliative prostatectomy can improve the quality of life obviously for non-cure patients with outlet obstructive symptoms.
