| Pregnancy-induced hypertension syndrome (PIH) is a species of multisystem disease,and it is special and frequent on pregnant women. Pregnancy-induced hypertension syndrome is a important reason to lead to the death of mother and infant.The pathogenesy of pregnancy-induced hypertension syndrome is not very clear.Recently someone find eclampsism patients presence insulin resistance (IR). Hyperinsulinemia may not only lead to descend of nitrogen monoxidum and metabolic disturbance of lipid but also reflect composition of PE2 and increase force of resistance of peripheral vascular.So hyperinsulinemia may step up blood pressure.These explain IR have some invariable effect in pathogenesis of eclampsism. Sex hormone binding globulin (SHBG) is a species of sterin haptoglobulin which is form from liver.The level of SHBG is an indirect index of balanate between androgen and estrogen.Not only hyperinsulinemia but also IR have intimate relationship with blood serum SHBG level.Insulin is very important to accommodate the metabolism of SHBG.Its heightening make SHBG derease and result in metabolic disorder of hormonal sex and dysbolism of sugar and fat.On the contrary these make IR further aggravate.SHBG have higher chemical force with andrusol and lower chemical force with E2.SHBG is transported to organiz as a regulator of androgen.So degrading of SHBG induces to heighten of free testosterone index(FTI).Androgen can strengthen blood vessel's responsibility to vasoactive substance and cut down the level of prostacyclin and increase the production of thromboxan(TX).It can increase platelet aggregation directly.Accordingly degrading of SHBG provoke typical vascular change of PIH.Overseas some reports indicate that degrade of SHBG of early trimester of pregnancy blood plasm may be high risk factor to trouble PIH.This experiment adopts radio-immunity technique to detect level of SHBG and insulin in blood plasm of eclampsism and analyses relationship between SHBG and IS to find relevance ofSHBG and eclampsism.We approach the contribution of SHBG in PIH through this channel to establish foundation for one step further to investigate pathogenesy and diagnose and intervention of PIH.Methods We gather 32 eclampsism patients and 30 normal pregnancy to draw-off venous blood on an empty stomach before pretherapy or antepartum. Apply oxidation enzyme to measure fasting blood glucose and radio-immunity to detect the level of insulin and SHBG.After calculation insulin resistance index, compare SHBG,insulin and insulin resistance index between experimental group and control group. Statistical analysis to make use of statistical package SPSS13.0.And undertake analysis effect of newborn body weight on the basis of light and heavy press and gestational weeks in eclampsism.Results1. Fasting blood glucose and insulin of eclampsism group are both higher than control group(P<0.05). Insulin resistance index of eclampsism group is higher than control group(P<0.05).SHBG of eclampsism group is lower than control group(P<0.05).2. Gestational weeks and newborn body weight of eclampsism group are lower than control group(P<0.05). Gestational weeks and newborn body weight of early onset in eclampsism group are lower than forme tardive,but the difference of newborn body weight is not significance. Gestational weeks and newborn body weight of severity in eclampsism group are lower than gently,and differences of both are significance.3. There are negative correlation between SHBG with IR and blood pressure in eclampsism.And there are positive correlation between SHBG with gestational weeks and newborn body weight in eclampsism.These dependability possess statistical significance.Conclusion:Breakdown of SHBG exist determinate dependability with IR in eclampsism.This maybe concerned with pathogenesy of eclampsism and effect to pregnancy outcome. |
