Background and objectiveIntracerebral hemorrhage (ICH) is a common disease in nervous system, the characteristic of which are high attack rate, disability rate and case fatality rate, and it influences survival and live standard for human being. There are two styles of neuron death: necrosis and apoptosis. Apoptosis involves the pathological process of secondary brain injury by gene regualte. It is significant that studying the regulation of apoptosis and how to inhibit apoptosis for promoting recovery of nervous function. So the changes of apoptosis expression with time in bilateral cerebral striatum of rats caused by intracerebral hemorrhage, the influence of early hematoma aspiration treatment on apoptosisb and neuroprotection of the operation will be investigated.Method1. In this experiment 108 mature male Sprague-Dawley rats were used. They were divided into four groups randomly: the normal group, the sham operation group, the ICH no treatment group, the hematoma aspiration after ICH group.2. ICH was induced by injection of collagenase IV into the caudate nucleus of rats. The shame operation group was induced by injection of saline instead of collagenase IV. The rats of hematoma aspiration group were injected with urokinase to lyse the hematoma after collagenase IV into the caudate nucleus for 5 h, then 1 h later, the clot was aspirated.3. The rats of all groups were killed on the corresponding time(6 h, 12 h,1 d, 3 d, 7 d, 11 d) and neurologic impairment score of each rat was measured before killing.4. The brain tissue sample were fixed in 4% paraformaldehyde (PFA) for 3 h at room temperature, paraffin-embedded after routine dehydration methods, made into 5μm cross sections.5. Terminal deoxynucleotidy1 transferase mediated dUTP nick-end-labeling method was applied to measure the apoptosis cells of nervous tissue.6. Bcl-2, Bax, Caspase-3 positive cells were observed by the immunohistochemistry method.Results1. There was no significant diversity between the normal group and the sham group on the positive cells numbers of apoptosis. Compared with sham group, TUNEL positive cells appeared in the ICH without treatment group, increasing at 1 d (p<0.05), peaking at 3 d (p<0.01), decreasing at 7 d,11 d (p<0.05).2. Compared with the sham group, Caspase-3, Bax positive cells appeared in the ICH without treatment group, increasing at 12 h (p<0.05), peaking at 1 d (p<0.01), and then decreasing at 3 d, 7 d, 11 d (p<0.05).3. Compared with the sham group, Bcl-2 positive cells appeared in the ICH without treatment group, decreasing at 12 h (p<0.05), peaking at 1 d (p<0.01), statistical significance of 3 d, 7 d, 11 d (p<0.05).4. Neurologic functional recovery of the hematoma aspiration group were better than the ICH without treatment group at 3 d (p<0.01), 7 d (p<0.05), 11 d (p<0.05).5. TUNEL positive cells in the hematoma aspiration group were less than the ICH without treatment group at 1 d, 3 d, 7 d, 11 d (p<0.05), and significant difference at 3 d (p<0.01).6. Caspase-3 positive cells in the hematoma aspiration group were less than the ICH without treatment group at 1 d, 7 d, 11 d (p<0.05), and at 3 d (p<0.01). Bax positive cells in the hematoma aspiration group were less than the ICH without treatment group at 1 d, 3 d (p<0.05), and at 7 d, 11 d (p<0.01). Bcl-2 positive cells in the hematoma aspiration group were higher than the ICH without treatment group at 1 d, 7 d, 11 d (p<0.05), and at 3 d (p<0.01).7. Between the score of neurofunction after ICH and the number of TUNELPositive eells,eorrelation eoeffieient 15 0.86.Conclusion1 .The neurologie imPairment after ICH maybe relevant to aPoPtotieInJUry·2 .The number of aPoPtosis eells and rats neurologie imPairment seoreafter ICH ean be redueed by early stereotaxis hematoma asPiration.
