Research For Survival Quality, Depression And Idea Of Suicide Of Uremia Maintaining Hemodialytic Patients

Objective: Our research includes the patients` nature states and disease character factors which including hemodialytic time, frequency and complications, analyzing the influences of difference factors to uremia hemodialytic patients` survival quality and depression etc. ,through standard psychological measurement instruments, providing effective clinical evidence of biopsychomedical model intervention and treatment to achieve the purpose of improving survival quality , lessening psychic burden.Methods: 102 patients diagnosed with chronic renal failure uremia and receiving hemodialysis in The North Hospital of Dalian Medical University No.2 Afiliated Hospital ,whose hemodialytic times were more than 3 months, excepting who have evident cardiac, cerebral and pulmonary diseases, taking 86 available at the end. Hemodialytic patients basal state examination, Generic Quality of Life Inventory -74(GQOLI-74), Self-rating Depression Scale(SDS),and ( SSI ) are used through questionnaire examination, clinical interview and history consulting. By establishing SPSS database, statictical methods that single factor analysis, chi square analysis, and multi-logistic regression analysis are applied. Results:1. There were 40 cases suffered depression in all 86 uremia hemodialytic patients, which 46.5 percentage is accounted. There were no significant differences in depressive incidence rate between variant sex,age,marriage and occupation(P>0.05), while a significant difference exists in which between variant education and income degree(P<0.05). There were significant differences in depressive score degree between the hemodialytictime, remain renal function , the degree of anemia and complications (P<0.05). Multi-logistic regression analysis show that the hemodialytic time and complications are the important factors of depression.2. The research found that there have the lowest scores in physical and psycho function in all the dimensionalities of survival quality. There have little influence of varient age, sex and occupation in survival quality , while have important influence of income, education degree, the hemodialytic time, complications and depressive state. Multi-logistic regression analysis shows that there have significant influences in income, hemodialytic time, complications and depression, which hemodialytic time, depression and complications have negative influence, while income have the positive .3. there were 15.12 percentage in uremia hemodialytic patients of suicide intention, which have no significant differences between variant age and marriage state(P>0.05), while a significant difference exists in which between variant age , education degree, depression, hemodialytic time and complications (P<0.05). There were significant difference in incidence rate and degree of depression between having idea of suicide and none, which is higher than the latter in incidence rate(P<0.05). Multi-logistic regression analysis shows that depression and complications are the main factors of suicide intention, the higher the depressive degree and the more the complications, the stronger the suicide intention.Conclusion:1. 86 cases uremia hemodialytic patients despondent formation rate is 46.5%, and the depression disease incidence rate in different literary standard and economical standard is remarkable, the hemodialytic time, the illness complication and so on, each kind of disease factors are remarkable to the depression influence, the disease factor is the main reason which causes depression to occur.2. The uremia hemodialytic patients have low life quality, and there have the lowest scores in physical and psycho function of all the dimensionalities of survival quality, which imply that the symptoms of uremia and psychological disorder are the main causes of survival quality. Increasing the survival quality, we must decrease the accidence of depression and the complications.3. 86 cases uremia hemodialytic patients formation rate is 15.12%, who receive high education,long time hemodialytic and have merge despondent and illness complications are easy to have the suicide thought, and improving the patient body function is allowed to decrease depression and the suicide thought to occur.4. The uremia hemodialytic patients have low life quality, higher incidence rate of depression and suicide intention than other patients, which is the important group of being take care of and interfering in. The research direction in the future should be discovering the effective biopsychosocial model treatment to increase their survival quality, physically and mentally health.
