Objective To explore the influence of alcohol consumption during Gliclazide treatment on efficacy in male patients with type 2 diabetes and the association of PPARypolymorphism and baseline of insulin index and blood lipid. Methods In order to elucidate genetic and environmental influence on individualized response to antihyperglycemic, from March 2004 to August 2005, a study on pharmacogenomics in type 2 diabetes was conducted in Haerbin,Beijing,Tianjin and Anhui province. The current study is part of it. In total, 721 subjects (392men, 329women) recruited to the prospective study cohort and 658 of them completed 57 days Gliclazide treatment. ECG and blood examination was performed at baseline and collection of clinical and epidemiological information was completed during treatment. We analyzed the data by methods of T test, ANOVA, multiple linear regression and multiple Logistic regression. Results (1) After 57 days therapy with Gliclazide, Natural Logarithmchange of HOMA-IR decreases 0.24 (P=0.29) , average decreases 0.11 μUmol/l~2 inlittle alcohol drinking group and 0.45 (P=0.04) ,0.19 μUmol/l~2 in over alcoholdrinking group. Natural Logarithm Change of HOMA-B decreases 0.26 (P=0.37) , average decreases 2.6 U/mol in little alcohol drinking group and 0.60 (P=0.04) , 6.0 U/mol in over alcohol drinking group. (2) The frequencies of three genotypes of PPARγ are CC genotype 33.43%, CT genotype 50.61%, TT genotype 15.96%,accorded with Hardy-Weinberg equilibrium(X~2=1.28, P=0.25).According to thestandard of China Hyperliponemia Prevention and Therapy Groups, we divided level of TC 9.0mmol/L and LDL 5.7mmol/L into normal and abnormal. Insulin resistance isthat HOMA-IR is above 2.25 μ Umol/l~2. Severe insulin resistance is that HOMA-IR isabove 3.60 μ Umol/l~2.
