Background:The inflammatory response in cardiac surgical patient is produced by the activation of complement ,cytokines and proteins, which are caused by the contact of blood with nonphysiological substance such as cardiopulmonary bypass duct ,ischemic-reperfusion,operation, and induced systemic inflammatory response syndrome in the end. Many factors activate inflammatory cells ,mainly Polymorphoneuclear neutrophil. Proinflammatory cytokines and anti-inflammatory cytokines are changed during cardiopulmonary.Object: study the relation of the change of amount of leukocyte and polymorphoneuclear neutrophil, and the concentration of IL-6,IL-10 and TNF-a.Methods: 10 rheumatic heart disease patient undergoing valves replace were selected eligible for the study.(group study).10 patient undergoing general thoracic operation were select for contrast(group contrast). The amount of leukocyte, polymorphoneuclear neutrophil and the pecent of polymorphoneuclear neutrophil in blood and the serum IL-6,IL-10 and TNF-a were measured before surgery,10 minutes after initiation of cardiopulmonary bypass ,at the termination of cardiopulmonary bypass, and 2 hours,6 hours,12 hours,1day,2days,3days,7days after cardiopulmonary bypass.While in contrast group, we measure the amount of leukocyte ,neutrophil and the pecent of neutrophil in blood and the serum IL-6,IL-10 and TNF-a before surgery,after open the thoracic ,close the thoracic,and 2 hours,6 hours,12 hours,1day,2days,3days,7 days after surgery. The serum IL-6,IL-10,TNF-a were measured by ELISA.Result: 1.After cardiopulmonary bypass, the amount of leukocyte and polymorphoneuclear neutrophil was increased remarkably.The similar changes are in the general thoracic group, but not so remarkably as group study. There are also different in the amount of leukocyte and polymorphoneuclear neutrophil between the double valve replacement group and the single valve replacement group. 2.
