| With the rapid development of Laparoscopic surgery in gynecologic clinic,it could have taken the place of most conventional surgeries,because of its minimal invasiveness.The minimally invasive surgery performed by laparoscopy is known to have less local trauma,smaller incision,less disturbance to abdominal cavity,shorter operative time,and less blood loss.Now,utilizing the preponderance of laparoscope in our departrment,we investigate the prognostic,psychologic status,immune responses caused by laparoscopic surgery.Objective:To analyze prognostic factors of cervical cancer,try to find some clues for prevention and clinic treatment of cervical cancer in future,and to provide scientific evidence for Laparoscopic-assisted radical vaginal hysterectomy (LARVH).Methods:The data of 214 hospitalized patients with cervical cancer in Xinan Hospital from July 1,2000 to July 1,2005 were reviewed,and follow-up was made via mails,phone-calls,and home-visiting in those patients.The end-points of one-year,three-year,or five-year survival were set in accordance with the days when cases were enrolled into this study.The factors which might impact on the patients'prognosis would include the general information of the patients themselves,the tumor data,and different treatment manners.Result:A total of 214 cases were enrolled in this study and 178 of them were followed up to the end-point of the study with16.8%(36cases) of loss to follow up.The average operative time was 189min(145-375 min),with a average blood loss of 289ml(120-750 ml),and an average 13.2 day's length of stay.The overall one-,three- and five -year survival rates of cervical cancer patients treated by LARVH were 93.94%,82.85%,79.91%,respectively.There was significant decrease in the survival rates among groups with high clinical stage,lymphatic metastasis.Clinical stage and lymphatic metastasis were correlated with survival rate.Conclusion:Laparoscopic surgery for cervical cancer is a safe and effective alternative to conventional RH.Clinical stage,lymphatic metastasis are independent prognostic factors of cervical cancer patients.Clinical stage and lymphatic metastasis are correlated with the prognosis and might be used to evaluate prognosis in the patients with cervical cancer. |
