Object:CO is an important gaseous messenger mediator that is supposed to be involved in the formation of irritable bowel syndrome.In vivo it was y synthesized under the catalysis of heme oxygenase (HO).In this study, the change of HO-CO System in the patient with irritable bowel syndrome was investigated.It's effect on the pathogenesis of irritable bowel syndrome were discussed.Methods:The rectal pressure, rectal visceral perception thresholds to balloon distension and the distribution of HO-1 and HO-2 proteins in the colonic tissues in 38 cases of IBS and 15 cases of normal control group were detected by SABC immunohistochemistry and perfusion catheter manometer.The slides were then analyzed with an Image Analyzing system to obtain the density(mean) of stained proteins. The concentrations of CO in the plasma were determined with chemical methods.Results:The rectal resting pressure,maximal squeezing pressure and defecation pressure in the patients between IBS and control group were not significant. Thresholds of first perception,defecation and maximal tolerable in the patients with diarrhea predominant were significantly lower than control group.Thresholds of first perception,defecation and maximal tolerable in the patients with constipation predominant were significantly higher than control group.CO concentration in the normal control group,the diarrhea predominant IBS (D-IBS),and the constipation IBS(C-IBS) were 16.65±0.92,19.69±1.07,16.14±1.05μmol/L respectively.The concentrations in D-IBS was significantly higher than that in the normal control group and in C-IBS.H0-1and HO-2 were expressed mainly in the colonic mucous membrane epithelia.The OD of the stained HO-1 protein in the normal control group,the diarrhea predominant IBS and the constipation IBS were 0.164±0.015,0.304±0.025,0.163±0.017 respectively.The OD values in D-IBS was significantly higher than that in the normal control group and in C-IBS.The OD of the stained HO-1 protein in the normal control group, the diarrhea predominant IBS and the constipation IBS were 0.319±0.019,0.316±0.023,0.314±0.019.It indicated that there were not differece detween IBS group and the normal control group.Conclusion:1. The rectal resting pressure,maximal squeezing pressure and defecation pressure in the patients between IBS and control group were not significant.It suggests that the patients with IBS may have not motor dysfunction in anorectum.2.
