Object: To improve the levels of iagnosing hilar bileduct carcinoma, and evaluate the focus on surgical treatmentof hilar bile duct carcinoma. Methods: Retrospect analysis of surgical management wasperformed on patients with hilar bile duct carcinoma in theNO.1 Hospital of Jilin Univercity between January 2000 andJanuary 2005.Results: There were 24 males and 15females in 39 cases, aged33 to 75 years(mean 52.5 years). The tumor of 16 cases wasresected. The other 17 cases, which were no resected weredone with internal or external drainage. Conclusion:It is still difficult to make early diagnosisin hilar bile duct carcinoma.Imaging examination played animportant role in detecting hilar bile duct carcinoma, andpreoperational assessing the respectability, of withultrasonography B and CT should be the first choice, and MRincluding MRI and MRCP was a kind of new, safe and sensitivemethod. Laparotomy must be performed in all patients whilethere were no contraindications. The surgical resectionsurpasses the other therapy in the long-term curative effect,If the tumor could not be resected, select drainage andsupport treatment should be chosed in order to prolongsurvival times and improve life quality. Radical resectionshould be accomplished if it is feasible. But complicationsafter the surgical operative resection were obvious higherthan other therapy. Therefore, the foreground of radicalresection is still very rigorous.
