Background: Premenstrual Syndrome (PMS) is a disease, which occurs before menstruation, affects women' s daily life and work, deals with spirit behaviors and physical behaviors. In traditional Chinese Medicine, there is no such a disease called like that. According to the clinical symptoms, we can conclude them to several diseases such as swelled pain of breast before menstruation, diarrhea before menstruation, edema before menstruation, headache before menstruation, fever before menstruation, body-ache before menstruation, stomatitis before menstruation, wheal before menstruation, spirit disturbed before menstruation and so on.In Modern Traditional Chinese Medicine studies, the distribution of syndromes of TCM in PMS are clear, and manifested as rule of disorder of Liver qi smoothing qi flow(58. 9%), syndromes of the liver qi invasion the Liver qi stagnation(27. 5%), flaming up of the Live fire(5.2%) and deficiency of the heart and spleen (3.3%). These four cover 95 percent, so there are main syndromes of TCM. Therapy deal with Liver has been recognized as the main method.Studies of these years have show that E2, P and PRL have important effects on the main syndromes of PMS. Neurotransmitter such as NE, E and 5-HT are parts of an important substance which is related to behavior and emotion.Many scholars try to combine disease to syndromes of TCM, analyses periodic changes of endocrines and neurotransmitter of PMS sufferers, carry through canonical study on every syndromes of TCM of PMS, so that they can have proof to go on studies on use herbs according to different syndromes of TCM.Objective: This study tried to investigate main clinical syndromes of PMS liver qi flow contradictorily, probe into its pertinency with endocrines ( E2, P, PRL) and neurotransmitter ( 5-HT, NE, E ). The aim was to find out an inner relation between the microcosmic and the macroscopical diagnosis, so thatcould enrich and expand TCM diagnostic methodologies.Methods: The study use "Daily Record of Severity of Problems " ( DRSP ) and "Form of Self-estimate in everyday syndromes in a menstrual cycle" contrived by Shan Dong University of Traditional Chinese Medicine. We studed representations of 149 cases of PMS Liver qi flow contradictorily, mensurated levels of E2, P, PRL, 5-HT, NE, E. The evaluation and statistics were done using Microsoft Excell, SPSS10. 0, T test, calculating data expressed x +SD.Results:1. PMS is a familiar disease in Gynecology, and the main syndrome of TCM is Liver qi flow contradictorily.2. Because PMS is a disease correlated with body and mind, its attack seems positive correlated with age, mostly happens between 18-23 years old women. The incidence of the disease of unmarried women is higher than married ones. The attack may somewhat related with educational background, but there' s no positive correlation.3. The course of PMS is long, and the syndromes can outbreak times without number. In the majority, the syndromes of TCM is Liver qi flow contradictorily appear in a week before menstrual flow and disappear before the menses stops, fidget, testiness or getting angry without any reason is the inevitable syndrome. Other symptoms arranged by times they appear: hypogastrium-ache before menstruation, absent-minded or ability descend before menstruation, swelled pain of breast before menstruation, headache before menstruation.4. PMS Liver qi flow contradictorily is somewhat correlated with E2, P, PRL, 5-HT, NE and E. During follicular period the levels of PRL, E2 and P are higher than normal. But during ovulatory period the levels of E2 and P are lower than normal, and the level of PRL is evidently shifted. During luteal period the level of PRL is higher than normal, E2 and P levels are lower than normal. 5-HT level is higher in the whole menstrual cycle. NE and E levers don't changed much in follicular period, but heightens in the other two periods.
