| Objective: To evalute the application of conventional MR Imaging and diffusion-weighted imaging (DWI) in occuping diseases of female pelvic.Materials and Methods: All images were performed in 1.5T supraconduction MR scanner (Eclipse, Marconi Medical System, USA), using body package flexile coil. â‘ 30 healthy volunteers were scanned using various b values (b=200,300,400, 500s/mm~2).The values of apparent diffusion coefficient (ADC) of bladder and uterus were measured for 3 times, then the mean values were computed.â‘¡60 patients were examined with conventional sequences and DWI.The b value was 300s/mm~2 which was based on the former. After scan, the ADC values of lesions were measured for 3 times, and the mean values were computed.Results: 1. The comparison among the different b values of healthy volunteers: the corresponding ADC values of b=200,300,400,500mm/s~2 had no statistic difference (P>0.05);2. 59 lesions were found in conventional MR images. The accuracy and specificity rate in diagnosis were higher than 90%;3. The comparison of ADC values of various uteri and ovary lesions: there was no statistic difference between the leiomyoma uteri and adenomyeosis (P=0.873);There was no statistic difference between the cystic component of thebenign and malignant ovary lesions (P=0.315);There was statistic difference between the solid component of the benign and malignant ovary lesion (P=0.039). Because the solid components of the benign and malignant lesions were discrepant to mormal distribution, percentile was used to get a reference value. P5=1476.230xl0'3mm2/s through the ADC values of the benign solid lesions was got.That the ADC value exceeded P5 suggested benign lesions, and that did not exceed suggested malignant lesions.The sensitivity of the reference value was 100% and specificity was 75%. /^ 1603.280* 10'3mm2/s of malignant solid lesions was got with the same method. That the ADC value exceeded P95 suggested benign lesions and if it did not exceed, it suggested malignant lesions. The sensitivity was 100% and the specificity was 40%. Because the sensitivity and specificity of 1476.230xl0'3mm2/s was higher than 1603.280xl0"3mm2/s, the value was chosend as the cutoff value of differenciating the benign from malignant lesions. Exceeding 1476.230* 10"3mm2/s will indicate benign lesions and those less than it will indicate malignant lesions.Conclusions: MRI can sensitively find lesions, and has enough value in the location, qualitation and quantitation of female pelvic lesions. Various b values of DWI do not influence the ADC values, and DWI can be used in differenciating benign from malignant tumors with solid component. |
