Application Of Fundus Photography In The Diagnosis And Curative Effect Evaluation Of Cyclodeviation

Purpose To investigate the clinical significance of objective status of ocular torsion in abnormity of oblique muscle before and after surgery with the method of fundus photography. Discuss its diagnosis and evaluation effect in cyclodeviations Methods Objective cyclodeviations were assessed by fundus photography before and after surgery. The photograph was transferred to a computer, and the forea— disc angle was measured by means of drawing picture software (Auto CAD R14) Objective cyclodeviations were measured in seventy normol person (140 eyes)without strabismus .Eighteen patients(36 eyes) with congenital superior oblique muscles palsy(CSOP),eleven patients(22 eyes)with primary inferior oblique muscles overacting (IOOA)underwent myotomy or partial myectomy of inferior oblique muscles,whose objective cyclodeviations were measured before surgery as well as 2~4 days after. Seven patients(14 eyes)with primary overacting of superior oblique muscles (SOOA)underwent myotomy of superior oblique muscles, whose objective cyclodeviations were measured before and after surgery. Two patients with vertical muscle palsy were measured before surgery. Subjective cyclodeviations were measured in ten patients with primary superior oblique muscles palsy by doublemaddox rod test (DMRT).The prism degree of A> V pattern were measured by synoptophore.Results The forea-disc angle of normol person is 5.235° ±2.631 ° right eye, 6.682° ±2.857° left eye , 11.917° ±4.243°totally .The difference between them is 3.113° ±2.113°. Sixteen out of eighteen patients with CSOP and nine out of eleven patients with IOOA showed excyclotropia. Seven patients with SOOA showed incyclotropia .One case of inferior rectus muscle palsy showed incyclotropia distinctively .One case of superior rectus muscle palsy showed excyclotropia distinctively. A pattern relate with incyclotropia .V pattern relate with excyclotropia . Objective cyclodeviations are going to normal after surgery. The more overaction of oblique muscle the more significant of V pattern the more significant change of forea-disc angle by surgery in patients with CSOP.Conclusions Fundus photography can describe the status of ocular torsion of cyclodeviation objectively, which provide credible evidence for clinical diagnosis, give guidance to the choice of surgery manner as well as value the effect of surgery.
