| objective To investigate the relationship between the plasma cytokines and intestinal bacteria-endotoxin translocation after gut barrier function was injured in severe acute pancreatitis rats.Methods SD rats were divided randomly into sham operation group(n=36) and SAP group (n=36). SAP animal model was developed in adult male rats by retrograde injection of 4% sodium taurocholate 1 ml/kg in biliopancreatiic duct. Morphology of pancreas and ileum were observed. The plasma levels of TNF-a,IL-6 and IL-10 were determined by ELISA. The plasma levels of DAO activities and LPS were examined at various time points. The rats of bacterial translocation to organs were also calculated. Results1. Histologic examination revealed no signs of pancreas and ileum, wheras all rats with SAP had parechymal hemorrhage and necrosis. The pancreatic sections showed parenchymal and pancreatic acini inflammatory cells infiltrate, significant hemorrhage and necrosis and fatty tissue necrosis after 24 hours, simultaneously, the ileum sections also displayed intestinal edema, hemorrhage, villous epithelium cells necrosis ,ulcer and neutrophil leukocytes infiltration.2. The plasma levels of TNF-a and IL-6 elevated significantly after SAP induction and the levels of IL-10 elevated significantly only after hour 6.3. Plasma DAO activities levels increased duing the early stage of SAP and decreased sharply after hour 24.4. Plasma LPS levels also increased significantly during the early stage of SAP and reached second climax at 48 hour.5. The rates of bacterial translocation to organs increased sharply at hour 24 and reach 58.3 percent at hour 72.6. There are a significant positive correlation between cytokines and intestinal bacteria-endotoxin translocation.Conclusions The levels of cytokines increased and gut barrier function was injured in the early stage of SAP. Cytokines may impaired the intestinal microcirculation leading to gut barrier damage, which could promote the intestinal bacteria-endotoxin translocation. Simultaneously, intestinal bacteria-endotoxin translocation also induced excessive release of cytokines potentiating the gut barrier damage, which may causesystemic inflammatory response syndrome and multiple organ disfunction syndrome. There was a significant positive correlation beween cytokines and intestinal bacteria-endotoxin translocation. Cytokines and intestinal bacteria-endotoxin translocation play a important role in the pathogensis of enterogenic infection in severe acute pancreatitis. |
