Objective: to evaluate the effect of extracranial-intracranial bypass (EC/IC bypass) on the treatment of ischemic cerebrovascular disease. Methods: 5 patients with occlusive carotid artery disease underwent modified EC/IC bypass surgery, preoperative and postoperative hemodynamic and clinical states were analyzed. Results: All patients no stroke happened during the follow-up period. Clinical symptoms were improved. Regional cerebral blood flow (rCBF) measured by single photon emission computed tomography (SPECT) showed that rCBF was increased post operatively. Conclusions: extracranial-intracranial bypass surgery can improve the rCBF and the clinical symptoms, can prevent complete stroke. Its long term effect needs further confirmation. Application of SPECT is helpful to evaluate the effect of the surgery.
