Objective To investigate the relationship between the variations of serum and urine type Ⅳ collagen (C-Ⅳ), laminine (LN) and type Ⅲ collagen (C-Ⅲ) and the degree of kidney lesion caused by hypertension .Methods 57 patients with essential hypertension without proteinuria and renal dysfunction in general examination and 57 controls were enrolled in this study . The concentrations of serum and urine C-Ⅳ, LN and C-Ⅲ of the two groups were detected and compared as well as the urine albumin and β2-microglobin . In addition , the associations of urine albumin and β2-microglobin with serum and urine C-Ⅳ, LN and C- Ⅲ in the hypertension group were analysised.Results In the hypertension group , serum and urine C-Ⅳ, LN and C-Ⅲ were significantly higher than those in the control group (P<0.05 -0.001) except urine C-Ⅳ. The excretion of urine albumin in hypertension was normal but had a positive correlation with the serum C-Ⅳ, LN and C-Ⅲ and urine C-Ⅳ and C-Ⅲ. The excretion of urine 62-microglobin in hypertension group enhanced significantly and had a positive correlation respectively with the serum and urine C- Ⅲ.Conclusion 1.There are increases of serum and urine C-Ⅳ, LN and C-Ⅲ in the early stage of hypertension without renal damage , particularly the increase of the C- Ⅲ in serum and urine. 2. In this stage , the normal excretion of albumin and the higher excretion of β2-microglobin of urine may mean that the injure of glomerulus isearlier than that of the tubulointerstitium.The increased serum and urine C-lll may represent the existence of tubular and interstitial lesion. 3. The result of the positive correlation between the urine albumin and C-IV, LN and C-lll dicrets that the increased urine C-IV, LN and C-lll may reflect the degree of glomerular damage . The result of the positive correlation between urine 62-microglobin and C- DI of serum urine prompt that the increased serum and urine C- DI may represent the existence of tubulointerstitial lesion. 4. The combining examination of urine C-ID andB2-microglobin may be utilized as the methed of detecting and diagnosing the early damage of the kidney in hypertension.
