Objective: To explore the biomechanical properties and analyze the clinical outcomes of lockingcompression plate for treating unstable sacral fractures. Methods: 3-dimension finite element models were used to simulate normal pelvis and the unstable sacralfractures suffered â…  section ,â…¡ section or â…¢ section sacral fracture respectively, and the ipsolateral pubiclimbs' fractures. The unstable sacral fractures fixed with one screw for the fracture of the superior pubiclimb and sacral screws, sacral bars or locking compression plate for the fractures of sacrum respectivelywere simulated. The fructures' mobility of the sacrum and pubis were calculated after the fixed pelviseswere given 500N axial load on 5th lumbar vertebra and the both femur were fixed .Furthermore, bonestress distribution of the fixed pelvises were compared with that of normal pelvis.Nine patients withunstable sacral fracture were treated with locking compression plate and followed-up.Their clinicoutcomes were estimated according to the Majeed standards. Results: Among the three fixing methods, the fixative effect of sacral screws is excellent and the effect ofsacral bars is similar to that of locking compression plate. When the pelvis was under axial load ,the bonestress distribution of the unstable pelvis fixed with one of three methods were similar to that of normalpelvis.the clinic outcomes of the 9 patients were excellent in 3,good in 4 ,fair in 2.Conclusion: The locking compression plate fixation treatment for the unstable sacral fractures is simpleand effective, with less trauma and complications.
