Purpose: To evaluate the value of CT for instructing the treatment of renal trauma and discuss the treatment of severe blunt renal trauma. Method: Retrospective analysis of 7 cases of severe renal trauma between 1995 to 2005, with the CT diagnosis of severe renal rupture (III~V grade) . 5 cases underwent conservative therapy;1 case appeared uncontrollable hemorrhagic shock and was performed nephrectomy, another case coexisting multiorgan injury adopted emergency surgery and nephrectomy too. Result: The sensibility of CT to renal trauma was 100% in this group, 5 conservative cases were cured without nephrogenic hypertension, hydronephrosis and other complications. The patients who taked nephrectomy recovered well after the surgery, without renal failure. Conclusion: CT should be preffered as renal injury, the severity of renal trauma that shows on CT could only be the consultation to decide the treatment. Clinical representation, such as the change of blood pressure, hematuria and local mass, may imply more significance. Conservative treatment could be adopted when life signs being observed carefully. Surgery should endeavor to retrieval the traumatic kidney, especially as the state is complex.
