Objective: To study the feasibility of anal-preserving procedure for super-low rectal cancer. Methods: 43 cases with super-low rectal cancer situated 3~5 cm from the anus in Dukes A,B,C from March 1997 to March 2004 were recruited to study the characteristic of clinical pathology,methods of operation, prognosis and function of anus after operation. Results: In 43 cases,21 were underwent operation of Miles,22 patients were underwent anal-preserving operation.No patient died for operation.All the patients were followed up from 1 year to 8 years.There were no statistical difference in 1,3 and 5 years survival rates and recurrence rates between this two groups. Postoperative anal continence was achieved with satisfaction in about 57.1%,76.2%,85% patients respectively after 3,6,12 months since operation,with 1 incontinence occur. Conclusion: The anal-preserving operation can be performed by surgeons with rich experience on the radical resection,the TME technique and the sphincter-preserving procedure for low rectal cancer. It can be selected in super-low rectal cancer with localized tumor,well differentiation and good general condition.
